目的观察分析彩色多普勒超声对乳腺肿块良恶性的鉴别诊断。方法随机抽取我院2015年1月至2015年12月进行治疗的86例乳腺肿块患者作为研究对象,本组患者均经过手术病理确诊,其中良性43例,恶性43例,对两组超声声像图进行比较,并观察肿块内部血流分级,检测血流参数阻力指数(RI值)。结果良性肿瘤患者在肿块形态、边缘、纵横比、后方衰减、有无钙化等和恶性肿块比较,结果具有统计学意义(P<0.05);43例良性肿块中包括有23例乳腺增生(含囊性增生和腺瘤样增生),18例纤维腺瘤,2例脂肪瘤;43例恶性肿块中包括有33例浸润性导管癌,7例原位癌,3例导管内乳头状癌。良性肿块血流分级主要为0-Ⅰ级,RI<0.7;恶性肿块血流分级主要为Ⅱ-Ⅲ级,RI>0.7。结论通过彩色多普勒超声对乳腺肿块检查,可确定肿块的大小、位置、形状、性质以及血流状况,能准确判断乳腺肿块的良恶性,在临床上具有重要的鉴别价值。
Objective To analyze the differential diagnosis of benign and malignant breast masses by color Doppler sonography. Methods Totally 86 cases of breast lump treated in our hospital from January 2015 to December 2015 were selected as the research object. All the patients were confirmed by operation and pathology, of which 43 cases were benign and 43 cases were malignant. Ultrasound imaging Graphs were compared and observed within the tumor mass flow classification, detection of blood flow parameters of resistance index (RI value). Results The results of benign tumor patients were statistically significant (P <0.05) in terms of tumor morphology, margins, aspect ratio, posterior attenuation, calcification, etc. There were 23 cases of benign breast masses including cyst hyperplasia Hyperplasia and adenomatous hyperplasia), 18 fibroadenomas and 2 lipomas. Forty-three malignant masses included 33 invasive ductal carcinomas, 7 carcinoma in situ and 3 ductal papillary carcinomas. The grade of benign mass was mainly grade 0-Ⅰ, with RI <0.7. The grade of malignant mass was grade Ⅱ-Ⅲ and RI> 0.7. Conclusion Color Doppler ultrasound examination of breast masses can determine the size, location, shape, nature and blood flow status of the mass, which can accurately determine the benign and malignant breast masses and has important clinical value.
