[目的]探讨活体肝癌经高强度聚焦超声(HIFU)治疗前后实时超声造影(CEUS)定量参数的变化特点,及其与肿瘤微血管密度(MVD)的相关性。[方法]选取30只新西兰大白兔制成VX2肝癌模型,随机分为对照组(15只,不作HIFU治疗)和治疗组(15只,作HIFU治疗)。行CEUS检查,获得并记录曲线增强上升斜率(AS)、开始增强时间(AT)、达峰时间(TTP)、峰值强度(PI)、曲线下面积(AUC),并与造影后通过免疫组织化学检查获得的MVD进行相关性分析。[结果]治疗组AS、PI、AUC较对照组明显下降(P<0.05);MVD较对照组明显减少(P<0.05);AT、TTP较对照组明显延迟(P<0.05)。CEUS定量参数中AS、PI、AUC分别和MVD呈正相关(P<0.05);AT、TTP分别和MVD呈负相关(P<0.05)。[结论]肝癌组织的CEUS定量参数与MVD具有高度的相关性,可反映肝癌组织微血管结构变化的情况并有助于判断HIFU对肝癌的疗效及预后。
[Objective] To investigate the changes of quantitative parameters of real-time contrast-enhanced ultrasonography (CEUS) before and after high-intensity focused ultrasound (HIFU) in vivo and its relationship with tumor microvessel density (MVD). [Methods] Thirty New Zealand white rabbits were selected to make VX2 hepatocellular carcinoma model and were randomly divided into control group (15 without HIFU treatment) and treatment group (15 with HIFU treatment). CEUS examination was performed to obtain and record the curves of ASR, TTP, peak intensity (PI) and area under the curve (AUC) Check the MVD obtained for correlation analysis. [Results] The AS, PI and AUC of the treatment group were significantly lower than those of the control group (P <0.05). The MVD of the treatment group was significantly lower than that of the control group (P <0.05); AT and TTP were significantly delayed compared with the control group (P <0.05). CE, PI and AUC were positively correlated with MVD (P <0.05), while AT and TTP were negatively correlated with MVD (P <0.05). [Conclusion] The CEUS quantitative parameters of hepatocellular carcinoma have a high correlation with MVD, which can reflect the change of microvessel structure in HCC and help to judge the curative effect and prognosis of HIFU on hepatocellular carcinoma.
