目的探讨肠道病毒71型((Enterovirus 71,EV71)感染后脑、心、肺等重要器官组织病理学改变。方法选用1日龄BALB/c乳鼠30只,随机分为3组,每组10只。第1组为EV71模型组,第2组为正常对照组,第3组为灭活EV71病毒对照组,采用脑内接种途径,分别接种EV71毒液20μL/只、生理盐水20μL/只和灭活EV71毒液20μL/只。每日观察病症变化,濒临死亡动物做组织解剖及病理学检查,采用EV71VP1片段引物做RT-PCR进行病毒核酸检测。结果接种EV71动物在3d后出现后肢瘫痪,5~7d动物濒临死亡。组织病理学显示EV71感染鼠乳大脑皮质锥体细胞核出现红染现象,小脑颗粒层变薄,脑组织局部出血、液化。心肌纤维水肿变性、溶解、萎缩,正常结构消失,间质出血,少量炎细胞浸润。肺脏明显出血,肺泡隔轻度淤血,可见含铁血黄素颗粒。脊髓前角细胞核红染,少量淋巴细胞浸润。在大脑锥体细胞检测到EV71核酸片段。感染组乳鼠心肌及对照组乳鼠脑组织、心肌中未检测到EV71核酸片段。结论脑内接种EV71可致1日龄乳鼠感染,并可致感染乳鼠瘫痪及死亡。病理学检查损伤主要累及脑、心、肺、脊髓等,其发病表现及组织病理改变与人类EV71感染危重症有着很高的一致性。
