目的了解2型糖尿病患者脂代谢异常情况,分析其血清的各项胆固醇及载脂蛋白含量的差异,以明确各项胆固醇或载脂蛋白与2型糖尿病糖代谢紊乱的相关性。方法分别测定60例经临床确诊的2型糖尿病患者、60例健康对照者血清中空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白(Apo)A1、A2、AB48、B100和E的水平,比较2型糖尿病患者以上各指标与健康对照者的差异,并进行相关性、多元逐步回归分析。结果相对于健康对照者,2型糖尿病患者的TC、TG、LDL-C水平显著偏高(P<0.01),HDL-C水平显著偏低(P<0.01),表现为脂代谢异常;血清载脂蛋白A2、B100、E水平均呈现不同程度的显著偏高(P<0.01)。多元逐步回归分析显示Apo B100、Apo B48、LDL-C和TG是空腹血糖水平的主要相关因素,表明载脂蛋白尤其是Apo B与糖代谢紊乱相关。结论 2型糖尿病患者机体内存在脂代谢异常,并且血清载脂蛋白水平与糖代谢紊乱情况密切相关。
Objective To understand the abnormal lipid metabolism in patients with type 2 diabetes mellitus and to analyze the differences of serum cholesterol and apolipoprotein contents in order to clarify the correlation between various cholesterol or apolipoproteins and type 2 diabetes mellitus. Methods The serum levels of fasting blood glucose (FBG), total cholesterol (TC), triglyceride (TG) and high density lipoprotein cholesterol (HDL-C) were determined in 60 clinically diagnosed type 2 diabetic patients and 60 healthy controls, LDL-C, Apo A1, A2, AB48, B100 and E in patients with type 2 diabetes mellitus were compared with those in healthy controls and the correlations were analyzed. Multiple stepwise regression analysis. Results The levels of TC, TG and LDL-C in type 2 diabetic patients were significantly higher than those in healthy controls (P <0.01), while HDL-C levels were significantly lower (P <0.01) The levels of lipoprotein A2, B100, E were significantly higher in different degrees (P <0.01). Multivariate stepwise regression analysis showed that Apo B100, Apo B48, LDL-C and TG were the main relevant factors of fasting blood glucose levels, indicating that apolipoproteins, especially Apo B, are associated with disorders of glucose metabolism. Conclusion Patients with type 2 diabetes have abnormal lipid metabolism, and serum apolipoprotein levels are closely related to the disorder of glucose metabolism.
