目的:探讨过去5年内昆山地区非妊娠糖尿病巨大胎儿发生的危险因素。方法:以2005~2009年在昆山市妇幼保健所登记的孕产妇及产后新生儿为研究对象,排除母亲孕前糖尿病和/或妊娠糖尿病因素,以巨大胎儿出生的孕周、民族为配对条件,采用1:1配比的病例对照研究,以此探究巨大胎儿发生的危险因素。结果:多因素调整后,条件Logistic回归分析结果显示,母亲年龄在32岁以上(OR=2.79,95%CI:1.45~5.39)、孕前体重在53kg以上、身高大于164cm(OR=2.08,95%CI:1.41~3.06),男孩(OR=1.51,95%CI:1.20~1.89)、父亲经常吸烟(OR=1.54,95%CI:1.13~2.10)等是巨大胎儿独立的危险因素;而父亲职业为干部或科技工作者(OR=0.61,95%CI:0.41~0.91)则是独立的保护性因素。结论:母亲生产年龄偏大、孕前体重偏高、身高偏高、男性胎儿、父亲经常吸烟等是巨大胎儿独立的危险因素;而父亲职业为干部或科技工作者则为巨大胎儿独立的保护性因素。
Objective: To explore the risk factors of non-gestational diabetes mega fetus in Kunshan in the past 5 years. Methods: Maternal and postpartum newborns registered in Maternal and Child Health Care Hospital of Kunshan City from 2005 to 2009 were selected as research objects. Maternal gestational diabetes and / or gestational diabetes factors were excluded. : 1 Proportional case-control study to explore the risk factors for the occurrence of huge fetuses. Results: Logistic regression analysis showed that the age of the mothers was over 32 years old (OR = 2.79, 95% CI: 1.45-5.39), body weight over 53kg and height over 164cm (OR = 2.08, 95% (OR = 1.51, 95% CI: 1.20-1.89), and fathers often smoking (OR = 1.54, 95% CI: 1.13-2.10) For the cadres or technologists (OR = 0.61, 95% CI: 0.41-0.91) were independent protective factors. Conclusion: The maternal age of birth is too large, high pre-pregnancy weight, height, male fetus, father often smoking is a huge fetal independent risk factors; father’s job as a cadre or technology workers are huge fetal independent protective factors .
