为进一步探讨内窥镜检查对鼻咽疾患的早期诊断价值,作者应用接触式显微内窥镜配以高清晰度电视摄像系统,结合活体染色,直接动态观察220例鼻咽正常或不同病变受检者鼻咽粘膜上皮的微观形态。接触式显微内窥镜最大放大倍数150倍,连接长焦摄像镜头后图像可放大400倍,能较清晰地显示粘膜上皮表层原位细胞的形态结构及分布排列情况,还能对粘膜毛细血管的形态及血细胞流动情况进行观察。初步观察结果显示:正常鼻咽粘膜上皮细胞排列较规则,多为柱状上皮细胞;鼻咽炎症的粘膜上皮细胞排列多不规则,脱落细胞较多,血管扩张,常见有鳞状细胞化生;鼻咽癌病灶染色不均,表层细胞形态及排列极不规则,微血管扩张、曲张,易出血。
