目的 :探讨氨茶碱对慢性阻塞性肺病 (COPD)运动耐力及其气体交换的影响。方法 :临床稳定期COPD患者 2 5例分别于氨茶碱 0 .2 5g静脉推注前后作极量运动试验 ,并用能量代谢仪连续测定呼吸参数及运动负荷。结果 :氨茶碱静脉推注后作功量、最大分时通气量 (·VEmax)、最大潮气量 (VTmax)和最大公斤体重氧耗量 (·VO2 max/kg)等显著增加 ,但相同运动负荷时的呼吸参数却无明显的变化。该药使用后 ,·VO2 max/kg增加程度与 ·VEmax(r =0 .85 4)和 ·VTmax(r=0 .6 5 6 )增加程度明显相关 (P >0 .2 )。结论 :氨茶碱能增加COPD患者的运动耐力 ,运动能力的增加可能与通气应答和 (或 )膈肌功能的改善有关。
Objective: To investigate the effect of aminophylline on exercise tolerance and gas exchange in patients with chronic obstructive pulmonary disease (COPD). Methods: A total of 25 stable COPD patients were tested for maximal exercise before and after 0.52 g intravenous injection of aminophylline, and the respiratory parameters and exercise load were measured continuously by energy metabolizer. Results: After intravenous injection of aminophylline, the maximal hourly ventilation volume (VEmax), maximum tidal volume (VTmax) and maximum kg body weight oxygen consumption (VO2 max / kg) increased significantly, but the same exercise No significant changes in respiratory parameters during loading. After administration of this drug, the increase of VO2 max / kg was significantly correlated with the increase of VEmax (r = 0.854) and VTmax (r = 0.666) (P> 0.2). CONCLUSION: Aminophylline increases exercise tolerance in COPD patients, and increased exercise capacity may be related to improvement of ventilatory response and / or diaphragmatic function.
