目的探讨不同糖耐量者甲状腺激素水平与胰岛素抵抗及胰岛功能的关系。方法采用化学发光法检测110例2型糖尿病患者(A组)、200例糖调节异常患者(B组)和124例糖耐量正常者(C组)的空腹胰岛素(FIns)、促甲状腺激素(TSH)、游离三碘甲腺原氨酸(FT3)和游离甲状腺素(FT4)水平。结果 A组和B组FT3为(4.29±0.37)pmol/L和(4.38±0.52)pmol/L,低于C组的(4.53±0.60)pmol/L(P<0.01);A组、B组TSH水平为(2.74±1.10)IU/ml和(2.41±1.00)IU/ml,高于C组的(2.30±0.91)IU/ml(P<0.05)。FT3水平与胰岛素抵抗指数(HOMA-IR)呈负相关(P<0.05)。结论 FT3水平与胰岛素抵抗密切相关,可能参与2型糖尿病的发生发展。
Objective To investigate the relationship between thyroid hormone levels and insulin resistance and islet function in patients with different glucose tolerance. Methods The levels of fasting insulin (FIns), thyrotropin (TSH), thyroid stimulating hormone (TSH) and thyroid stimulating hormone in 110 type 2 diabetic patients (group A), 200 patients with abnormal glucose regulation (group B) and 124 normal glucose tolerance patients (group C) ), Free triiodothyronine (FT3) and free thyroxine (FT4). Results The FT3 in groups A and B was (4.29 ± 0.37) pmol / L and (4.38 ± 0.52) pmol / L, respectively, lower than that in group C (4.53 ± 0.60) pmol / L TSH levels were (2.74 ± 1.10) IU / ml and (2.41 ± 1.00) IU / ml, higher than those in group C (2.30 ± 0.91) IU / ml (P <0.05). There was a negative correlation between FT3 level and insulin resistance index (HOMA-IR) (P <0.05). Conclusion FT3 level is closely related to insulin resistance and may be involved in the development of type 2 diabetes.
