目的探讨老年冠心病心力衰竭患者的N-末端脑钠肽前体(N-terminal pro brain natriuretic peptide,NT-pro BNP)、C反应蛋白(C-reactive protein,CRP)水平与左室射血分数(left ventricular ejection fraction,LVEF)的相关性。方法选择2012年2月—2015年6月在本院住院的116例老年冠心病心力衰竭患者作为观察组,纳入同期58例健康体检者作为对照组,比较两组的NT-pro BNP、CRP、LVEF值,并进行相关性分析。计量资料比较采用t检验及方差分析,两两比较采用LSD法,计数资料比较采用χ2检验,P<0.05为差异有统计学意义。结果两组研究对象NT-pro BNP、CRP及LVEF[(5 059.4±5 125.3)ng/L、(22.3±15.8)mg/L、(44.8±4.7)%与(297.4±97.2)ng/L、(5.3±0.3)mg/L、(57.4±2.8)%]比较,差异均有统计学意义(均P<0.05)。观察组患者NT-pro BNP、CRP与LVEF值呈负相关(r=-0.394、-0.431,均P<0.05);心功能分级与CRP水平呈正相关(r=0.545、0.495,均P<0.05),LVEF值与心功能分级呈负相关(r=-0.725,P<0.05)。结论血清NT-pro BNP、CRP水平与冠心病心力衰竭患者心功能分级呈正相关,与LVEF值呈负相关。
Objective To investigate the relationship between the levels of N-terminal pro brain natriuretic peptide (NT-pro BNP), C-reactive protein (CRP) and left ventricular ejection fraction (left ventricular ejection fraction, LVEF) correlation. Methods A total of 116 elderly patients with CHD who were hospitalized from February 2012 to June 2015 in our hospital were enrolled in this study. 58 healthy subjects were enrolled in the study. NT-pro BNP, CRP, LVEF value, and conduct correlation analysis. Measurement data were compared using t test and analysis of variance, two by two comparison using LSD method, the count data were compared using the χ2 test, P <0.05 for the difference was statistically significant. Results The levels of NT-pro BNP, CRP and LVEF in the two groups were significantly higher than those in the control group (5 059.4 ± 5 125.3 ng / L, 22.3 ± 15.8 mg / L, 44.8 ± 4.7% vs 297.4 ± 97.2 ng / L, (5.3 ± 0.3) mg / L, (57.4 ± 2.8)%], the differences were statistically significant (all P <0.05). The levels of NT-pro BNP and CRP in the observation group were negatively correlated with LVEF (r = -0.394, -0.431, all P <0.05). The grade of cardiac function was positively correlated with CRP level (r = 0.545,0.495, There was a negative correlation between LVEF and cardiac function (r = -0.725, P <0.05). Conclusion Serum levels of NT-pro BNP and CRP are positively correlated with heart function grading in patients with coronary heart disease and negatively correlated with LVEF.
