目的观察不同糖代谢状态下血清脂联素和游离脂肪酸的(FFA)的变化,以探讨其在糖尿病发生发展中的作用。方法测定25例正常糖耐量(NGT)组、27例糖耐量受损(IGT)组和25例初诊2型糖尿病(T2DM)组血清脂联素和FFA水平,分析其对胰岛素抵抗(HOMA-IR)和胰岛B细胞功能(HBCI)的影响。结果 (1)IGT组和T2DM组空腹血清脂联素低于NGT组,血清FFA明显高于NGT组,差异均有统计学意义(P<0.05)。(2)空腹血清脂联素与HOMA-IR显著负相关(r=-0.542,P<0.01),与HBCI无相关性。(3)血清FFA与HOMA-IR呈显著正相关(r=0.646,P<0.01),与HBCI呈负相关(r=-0.244,P<0.05)。结论 IGT和T2DM患者血清脂联素降低,FFA明显增高,并与HOMA-IR、HBC相关,提示血清脂联素和FFA可能参与构成T2DM的病理生理基础,包括IR和B细胞功能障碍。
Objective To observe the changes of serum adiponectin and free fatty acid (FFA) under different glycometabolism status in order to investigate its role in the development of diabetes mellitus. Methods Serum levels of adiponectin and FFA were measured in 25 patients with normal glucose tolerance (IGT), 27 with impaired glucose tolerance (IGT) and 25 with newly diagnosed type 2 diabetes (T2DM). The levels of insulin resistance (HOMA-IR ) And islet B cell function (HBCI). Results (1) Fasting serum adiponectin in IGT group and T2DM group was lower than NGT group, serum FFA was significantly higher than NGT group, the difference was statistically significant (P <0.05). (2) Fasting serum adiponectin was negatively correlated with HOMA-IR (r = -0.542, P <0.01), but not with HBCI. (3) There was a significant positive correlation between serum FFA and HOMA-IR (r = 0.646, P <0.01), but negatively correlated with HBCI (r = -0.244, P <0.05). Conclusions Serum levels of adiponectin and FFA are significantly increased in patients with IGT and T2DM, and are associated with HOMA-IR and HBC, suggesting that serum adiponectin and FFA may participate in the pathophysiological basis of T2DM, including IR and B cell dysfunction.
