目的探讨患阵发性室上性心动过速飞行人员的心血管功能状态,评价患阵发性室上性心动过速飞行人员的医学结论。方法对1994年7月至1997年6月期间连续住院的13例患阵发性室上性心动过速的飞行人员进行常规心电图和24 h动态心电图监测及有关的血液动力学检查,如超声心动图、平板运动试验,必要时行冠状动脉造影检查等,评价阵发性室上性心动过速患者的心功能,了解是否存在血液动力学改变和是否存在器质性心脏病。结果24 h动态心电图检查显示13例飞行人员均有阵发性室上性心动过速。经心血管系统全面检查发现,其中合并有冠心病1例,扩张性心肌病1例,预激综合症2例,有明确的血液动力学改变者3例,其余6例未发现明显异常。结论根据临床、心电生理学和心功能检查,提出对患阵发性室上性心动过速的飞行人员进行医学鉴定时,应视其是否合并严重的心脏病和阵发性室上性心动过速、是否影响心脏功能而定。如果合并器质性心脏病或有严重的心功能障碍者应予于停飞;如果无合并器质性心脏病,也无影响心脏功能情况者可考虑继续飞行。
Objective To investigate the cardiovascular status of patients with paroxysmal supraventricular tachycardia and to evaluate the medical conclusions of patients with paroxysmal supraventricular tachycardia. Methods Thirteen patients with paroxysmal supraventricular tachycardia who were hospitalized consecutively from July 1994 to June 1997 underwent routine electrocardiogram and 24-h Holter monitoring and related hemodynamic tests such as echocardiography Figure, treadmill exercise test, if necessary, coronary angiography, evaluation of patients with paroxysmal supraventricular tachycardia heart function, to understand the existence of hemodynamic changes and the existence of organic heart disease. Results 24 h Holter examination showed that all 13 pilots had paroxysmal supraventricular tachycardia. A comprehensive examination of the cardiovascular system found that there was 1 case of coronary heart disease, dilated cardiomyopathy in 1 case, pre-excitation syndrome in 2 cases, 3 cases of definite hemodynamic changes, the remaining 6 cases found no significant abnormalities. Conclusion According to the clinical, electrophysiological and cardiac function tests, the proposed medical assessment of patients with paroxysmal supraventricular tachycardia should be considered whether it is combined with severe heart disease and paroxysmal supraventricular tachycardia Speed, depending on whether the heart function.
