目的探讨脑脊液寡克隆区带(CSF-OCB)检测在神经系统疾病,尤其是多发性硬化中的应用。方法 259例神经系统疾病患者分为三组,分别是神经系统炎性脱髓鞘疾病(IDD)组99例(包括多发性硬化65例,视神经脊髓炎18例,吉兰-巴雷综合征16例),神经系统炎性疾病(NID)组99例和周围神经病组61例。应用等电聚焦联合免疫固定的方法检测受试者血清及脑脊液中OCB,并进行对比分析。结果 (1)CSF-OCB阳性率在IDD、NID和周围神经病组中分别为35.35%、34.34%和6.56%,周围神经病组阳性率明显低于IDD和NID组。(2)CSF-OCB阳性率在多发性硬化、视神经脊髓炎和吉兰-巴雷综合征组分别为49.23%、11.11%和6.25%,多发性硬化组CSF-OCB阳性率显著高于其他脱髓鞘疾病组,但与NID组比较无显著差异(χ2=3.616,P>0.05)。结论 CSF-OCB的检测可以作为多发性硬化和NID的辅助诊断指标,并且有必要寻找更加敏感的诊断指标。
Objective To investigate the application of cerebrospinal fluid oligoclonal band (CSF-OCB) detection in nervous system diseases, especially in multiple sclerosis. Methods Totally 259 patients with neurological diseases were divided into three groups: 99 cases of neurodegenerative inflammatory demyelinating disease (IDD) group (including 65 cases of multiple sclerosis, 18 cases of optic neuromyelitis, 16 cases of Guillain-Barre syndrome ), 99 cases of nervous system inflammatory disease (NID) and 61 cases of peripheral neuropathy. The serum and cerebrospinal fluid (OCB) levels in subjects were examined by isoelectric focusing and immunofixation, and compared. Results (1) The positive rate of CSF-OCB was 35.35%, 34.34% and 6.56% respectively in IDD, NID and peripheral neuropathy group. The positive rate of CSF-OCB in peripheral neuropathy group was significantly lower than that in IDD and NID group. (2) The positive rate of CSF-OCB in multiple sclerosis, optic neuromyelitis and Guillain-Barre syndrome group was 49.23%, 11.11% and 6.25% respectively. The positive rate of CSF-OCB in multiple sclerosis group was significantly higher than that in other groups Myelin disease group, but no significant difference compared with the NID group (χ2 = 3.616, P> 0.05). Conclusion The detection of CSF-OCB can be used as an auxiliary diagnostic indicator of multiple sclerosis and NID, and it is necessary to find more sensitive diagnostic indicators.
