以单纯眼球运动神经麻痹为主要表现的脑卒中很罕见,过去对其发病原因认识不够,最近文献提示脑干内损害可能是重要原因。本文报道经MRI或CT证实为脑干内损害的脑卒中7例。男5例,女2例,年龄38～76岁,中值年龄52岁。卒中的危险因素包括:高血压4例,吸烟1例,高血脂1例,冠心病1例,脑血管畸形1例。症状:7例病人均有复视,伴有头晕4例、面部麻木4例、头痛2例、共济失调和手臂活动不便2例、嗜睡1例。体征:动眼神经麻痹者5例,表现为上睑下垂3例,其中1例为双侧性,瞳孔改变者2例,动眼神经支配的眼肌无力者4例,仅下直肌不全麻痹者1例,垂直上视麻痹者2例,垂直下视麻痹者1例。滑车神经麻痹者1例,表现为单
Stroke is the most common type of stroke characterized by simple motor nocturnal palsy. In the past, the cause of the disease was not well understood. Recent literature suggests that damage may be an important cause of brain stem damage. This article reports 7 cases of cerebral infarction confirmed by MRI or CT as brain stem damage. 5 males and 2 females, aged 38 to 76 years, median age 52 years. Risk factors for stroke included 4 cases of hypertension, 1 case of smoking, 1 case of hyperlipidemia, 1 case of coronary heart disease and 1 case of cerebrovascular malformation. Symptoms: All 7 patients had diplopia with 4 cases of dizziness, 4 cases of facial numbness, 2 cases of headache, 2 cases of ataxia and immobility and 1 case of drowsiness. Signs: oculomotor nerve paralysis in 5 cases, manifested as ptosis in 3 cases, including 1 case of bilateral, pupil changes in 2 cases, oculomotor nerve domination of the eye weakness in 4 cases, only the lower rectus abdominis paralysis 1 case of vertical paralysis in 2 cases, vertical paralysis of 1 case.
