目的探讨星状神经节阻滞联合吗啡对晚期肿瘤患者疼痛缓解和生活质量的影响。方法选取2014年3月至2016年12月间秦皇岛军工医院收治的90例晚期肿瘤患者,根据治疗方法不同将患者分为研究组与对照组,每组45例。对照组患者采用口服吗啡片治疗,研究组患者采用星状神经节阻滞联合吗啡治疗。比较两组患者治疗前后疼痛和生活质量的变化。结果治疗后,两组患者VAS评分均较治疗前降低,差异均有统计学意义(均P<0.05)。研究组患者治疗15天及30天时吗啡使用量均低于对照组患者,差异均有统计学意义(均P<0.05)。治疗后,两组患者的疼痛自我效能量表(PSE)、功能性自我效能感量表(FSE)和应对方式自我效能感量表(CSE)评分均较治疗前升高,差异均有统计学意义(均P<0.05),同时,研究组患者上述各指标得分均比对照组高,差异均有统计学意义(均P<0.05)。治疗前,两组患者生活质量总分比较,差异无统计学意义(P>0.05)。治疗后,研究组患者生活质量总分高于治疗前,且高于对照组患者,差异均有统计学意义(均P<0.05)。结论星状神经节阻滞联合吗啡可减轻晚期肿瘤患者的疼痛,改善和提高患者生活质量,值得临床推广。
Objective To investigate the effect of stellate ganglion block combined with morphine on pain relief and quality of life in patients with advanced cancer. Methods Ninety patients with advanced cancer who were admitted to Qinhuangdao Military Hospital from March 2014 to December 2016 were selected. The patients were divided into study group and control group according to different treatment methods, with 45 cases in each group. Patients in the control group were treated with oral morphine, and patients in the study group were treated with stellate ganglion and morphine. The changes of pain and quality of life before and after treatment were compared between the two groups. Results After treatment, the VAS scores of both groups were significantly lower than those before treatment (all P <0.05). Morphine consumption in study group was lower than that in control group on the 15th day and the 30th day (all P <0.05). After treatment, the scores of PSE, FSE and CSE in both groups were significantly higher than those before treatment, with statistical differences (All P <0.05). At the same time, the score of each index in the study group was higher than that in the control group (all P <0.05). Before treatment, the two groups of patients with total quality of life scores, the difference was not statistically significant (P> 0.05). After treatment, the total score of quality of life in study group was higher than before treatment and higher than that in control group (all P <0.05). Conclusion Stellate ganglion block combined with morphine can reduce the pain of patients with advanced cancer, improve and improve the quality of life of patients, it is worth clinical promotion.
