目的:了解某医院门诊用药情况,分析不合理用药处方,促进临床用药安全。方法:采用回顾性调查方法,随机抽取某院2014年1~6月每月200张门诊处方,对其不合理用药处方进行归纳分析。结果:不合格处方156张,占总处方的13.0%。其中不规范处方75张(占48.07%)、不适宜处方74张(占47.44%)、超常处方7张(占4.49%);主要表现为处方开具不规范、处方药物与临床诊断不相符、用法用量不适宜及联合用药不适宜等。结论:通过处方点评,查找医师处方存在的不合理用药现象,在医务部门的干预和药师用药指导下,有利于促进临床药物应用的规范安全。
Objective: To understand the situation of outpatient medication in a hospital, analyze unreasonable medication prescription and promote clinical medication safety. Methods: A retrospective survey method was used to randomly select 200 outpatient prescriptions per month from January to June 2014 in a hospital, and to summarize their irrational prescriptions. Results: There were 156 unqualified prescriptions, accounting for 13.0% of the total prescriptions. Among them, 75 were non-standard prescriptions (accounting for 48.07%), 74 were not suitable for prescriptions (47.44%), 7 were prescriptions for over-compliance (4.49%). The main manifestations were irregular prescriptions, inconsistent prescriptions and clinical diagnosis The amount of inappropriate and combined medication is not suitable. Conclusion: Through the prescription reviews, it is helpful to find out the unreasonable medication existing in the prescriptions of doctors. Under the guidance of the medical department and the pharmacist, it is helpful to promote the standard and safety of the clinical drug application.
