目的探讨高血压早期患者血压变异率(BPV)与动脉弹性踝-臂指数(ABI)的年龄差异。方法未经服药治疗的高血压早期患者68例分为两组:A组41例,年龄<60岁;B组27例,年龄≥60岁。依据24-h动态血压测定不同时间多次血压的脉压(PP)及其均值标准差(SD)来确定BPV;同时采用全自动动脉弹性检测仪检测ABI。结果 B组24-h平均脉压、日间平均脉压、夜间平均脉压和夜间收缩压差值SD均高于A组[(63.4±17.4)mmHg vs.(51.4±6.0)mmHg、(63.6±17.0)mmHg vs.(51.7±6.2)mmHg、(61.4±21.0)mmHg vs.(49.4±8.0)mmHg和(11.2±3.7)mm Hg vs.(8.9±3.3)mm Hg](P<0.05)。B组ABI异常发生率也高于A组(51.9%vs.24.4%)(P<0.05)。结论≥60岁高血压早期患者的24-h平均脉压、日间平均脉压、夜间平均脉压、夜间收缩压差值及ABI异常率均高于<60岁者。
Objective To investigate the differences of blood pressure variability (BPV) and arterial ankle-brachial index (ABI) in patients with early-stage hypertension. Methods 68 cases of early treatment of hypertension without hypertension were divided into two groups: A group of 41 cases, aged <60 years; B group 27 cases, ≥ 60 years of age. BPV was determined according to the 24-h ambulatory blood pressure (PPV) and the mean standard deviation (SD) of multiple blood pressure at different time points. At the same time, ABI was detected by automatic arterial elasticity tester. Results The mean 24-h pulse pressure, average daytime pulse pressure, nighttime mean pulse pressure and nighttime systolic pressure difference in group B were significantly higher than those in group A [(63.4 ± 17.4) mmHg vs. (51.4 ± 6.0) mmHg, 63.6 ± 17.0 mmHg vs. 51.7 ± 6.2 mmHg vs. 61.4 ± 21.0 mmHg vs. 49.4 ± 8.0 mmHg vs 11.2 ± 3.7 mmHg vs. 8.9 ± 3.3 mmHg, respectively (P <0.05) . The incidence of abnormal ABI in group B was also higher than that in group A (51.9% vs.24.4%) (P <0.05). Conclusion The average 24-h pulse pressure, daytime mean pulse pressure, nighttime mean pulse pressure, nocturnal systolic pressure and ABI abnormalities were higher in patients with early-stage hypertension than 60 years old.
