目的评价浙江省2011年疑似预防接种异常反应(AEFI)监测系统运转情况和预防接种的安全性。方法通过全国AEFI监测系统和中国免疫规划监测信息管理系统收集浙江省2011年报告的AEFI个案数据及接种数据,采用描述性分析方法进行分析。结果 2011年浙江省AEFI监测系统共报告个案1615例,全省县级报告覆盖率达到100.00%;AEFI报告发生率为85.93/100万;<1岁组占38.76%,<2岁组占26.29%,男女性别比为1.59∶1;报告个案数较多的月份为4—8月。报告的AEFI中,一般反应占75.54%,异常反应占20.25%,接种事故占0.12%,偶合症占3.78%,心因性反应占0.25%,待定占0.06%。84.71%的AEFI个案均发生在接种后当天或1天之内。报告的AEFI涉及的疫苗,报告数前3位的是无细胞百白破疫苗、白破疫苗及麻疹风疹联合疫苗,报告发生率前3位的是23价肺炎疫苗(428.47/100万)、白破疫苗(347.56/100万)及百白破疫苗(168.06/100万)。2011年报告死亡5例,其中4例为偶合反应,1例为异常反应。结论 2011年全省AEFI监测系统的敏感性和数据质量有较大提高。2011年AEFI发生率为8093/100万,其中年龄以2岁以内儿童为主,与儿童免疫程序年龄分布相一致;发生时间大多在接种疫苗后1天内,AEFI报告发生率均在预期发生范围内。
Objective To evaluate the operational status of vaccinated suspected anaphylaxis (AEFI) surveillance system in Zhejiang Province in 2011 and the safety of vaccination. Methods AEFI case data and inoculation data reported by Zhejiang Province in 2011 were collected through the national AEFI monitoring system and China Immunization Monitoring Information Management System. The descriptive analysis method was used to analyze the data. Results A total of 1615 cases were reported by the AEFI monitoring system in Zhejiang Province in 2011. The coverage rate of county-level reporting in the province reached 100.00%. The incidence of AEFI was 85.93 / 100 million. The incidence of AEFI was 38.76% in the group of 1 year old and 26.29% , The male-to-female ratio was 1.59: 1; the months with the largest number of reported cases were from April to August. Among the reported AEFIs, 75.54% were normal, 20.25% were abnormal, 0.12% were vaccinated, 3.78% were coupled, 0.25% were psychogenic, and 0.06% were undetermined. 84.71% of AEFI cases occurred on the day after inoculation or within 1 day. The reported AEFI vaccines, the top three reported acellular diphtheriae vaccine, diphtheria vaccine and measles rubella combination vaccine, the top three reported a 23-valent pneumonia vaccine (428.47 / 100 million), white Vaccinated vaccine (347.56 / 100 million) and diphtheriae vaccine (168.06 / 100 million). Five deaths were reported in 2011, of which four were coupled and one was abnormal. Conclusion In 2011, the sensitivity and data quality of the AEFI monitoring system in the province have greatly increased. The incidence of AEFI in 2011 was 8093/100 million, of which children were mostly within 2 years of age and consistent with the age distribution of childhood immunization programs; the incidence of AEFI was most likely to occur within 1 day after vaccination, most of which occurred .
