目的观察来曲唑治疗多囊卵巢综合征(polycystic ovarysyndrome,PCOS)并不孕的临床效果。方法选择2008年6月—2010年6月收治的PCOS并不孕患者80例,随机分为治疗组和对照组各40例,于月经周期第3天开始,治疗组给予来曲唑片5 mg/d,对照组给予橼酸氯米芬胶囊100 mg/d。两组均用药5 d。当优势卵泡>18 mm时给予HCG 10 000 U肌内注射,比较两组卵泡发育情况、排卵情况、卵巢过度刺激综合征(ovariy hyperstimulation syndrome,OHSS)发生率、黄素化未破裂卵泡综合征(luteinized unruptured follicle syndrome,LUFS)发生率、6个月内妊娠率及HCG注射日子宫内膜厚度。结果治疗组出现优势卵泡30例,占75.0%,其中,单卵泡27例,2个卵泡3例。对照组出现优势卵泡26例,占65.0%,其中,单卵泡18例,2个卵泡6例,2例出现10个以上卵泡而放弃HCG注射,两组比较差异无统计学意义(P>0.05)。优势卵泡数及成熟卵泡数、排卵率、6个月内妊娠率及子宫内膜厚度,两组比较差异均有统计学意义(均P<0.05)。结论来曲唑具有良好的促排卵作用,是治疗PCOS首选药物,值得临床推广使用。
Objective To observe the clinical effect of letrozole in treating infertility with polycystic ovarysyndrome (PCOS). Methods 80 cases of PCOS infertile patients who were admitted to our hospital from June 2008 to June 2010 were randomly divided into treatment group (n = 40) and control group (n = 40), on the third day of the menstrual cycle. The treatment group was given letrozole 5 mg / d, the control group was given clomiphene citrate capsules 100 mg / d. Both groups were treated for 5 days. HCG 10 000 U was intramuscularly injected when the dominant follicle was> 18 mm. The follicular development, ovulation, incidence of ovarian hyperstimulation syndrome (OHSS), luteinized unruptured follicle syndrome follicle syndrome, LUFS), pregnancy rate within 6 months and endometrial thickness on HCG injection day. Results In the treatment group, there were 30 dominant follicles, accounting for 75.0%. Among them, 27 cases were single follicles and 3 cases were 2 follicles. In the control group, there were 26 dominant follicles, accounting for 65.0%. Among them, 18 cases of single follicles, 6 follicles of 2 follicles and 2 cases of follicles of more than 10 follicles gave HCG injection. There was no significant difference between the two groups (P> 0.05) . The number of dominant follicles and mature follicles, ovulation rate, pregnancy rate within 6 months and endometrial thickness were significantly different between the two groups (all P <0.05). Conclusion Letrozole has a good ovulation promoting effect and is the first choice for PCOS treatment, which is worthy of clinical promotion.
