目的研究卵巢功能早衰患者骨质疏松与机体甲状旁腺素(PTH)、25-羟基维生素D(25-OH-VD)和骨钙素(BGP)水平的相关性。方法选择2012年7月至2015年5月在安康市中心医院接受治疗的卵巢功能早衰合并骨质疏松患者100例,其中绝经年龄<36岁者48例,绝经年龄36~39岁者52例,另选同期在进行健康体检的52例女性作为对照组。对比各组PTH、25-OH-VD和BGP等指标水平,分析卵巢功能早衰患者PTH、25-OH-VD和BGP与腰椎(L_(2~4))骨密度(BMD)的相关性。结果绝经年龄<36岁组与绝经年龄36~39岁组的左髋部BMD、腰椎BMD均显著低于对照组(t值分别为7.200、7.337、7.178、5.199,均P<0.05),绝经年龄<36岁组与绝经年龄36~39岁组间比较左髋部BMD、腰椎BMD差异无统计学意义(t值分别为0.000、1.326,均P>0.05)。绝经年龄<36岁组与绝经年龄36~39岁组的唧水平均显著高于对照组(t值分别为2.283、2.165,均P<0.05),25-OH-VD水平均显著低于对照组(t值分别为8.340、8.247,均P<0.05),而各组的BGP水平相比,差异无统计学意义(F=1.094,P>0.05)。腰椎BMD与PTH呈显著负相关(R=-0.549,P<0.05),而与25-OH-VD呈显著正相关(R=0.507,P<0.05)。结论卵巢功能早衰患者发生骨质疏松与机体PTH水平升高以及25-OH-VD水平降低密切相关。
Objective To investigate the relationship between osteoporosis and parathyroid hormone (PTH), 25-OH-VD and BGP in patients with premature ovarian failure. Methods 100 patients with premature ovarian failure and osteoporosis who underwent treatment at Ankang Central Hospital from July 2012 to May 2015 were selected. Among them, 48 were in menopausal age <36 years, 52 were in menopause age 36-39 years, In the same period, 52 women who took part in a physical examination were selected as the control group. The levels of PTH, 25-OH-VD and BGP in each group were compared to analyze the correlation between PTH, 25-OH-VD and BGP and BMD in lumbar spine (L_ (2-4)). Results The BMD of lumbar spine and the BMD of lumbar spine were significantly lower in the menopausal age group <36 years and the menopausal age group 36-39 years old (t = 7.200, 7.337, 7.178, 5.199, respectively, P <0.05) There were no significant differences in BMD and BMD of lumbar spine between the 36-year-old group and the 36-39-year-old menopausal group (t = 0.000 and 1.326, respectively, P> 0.05). Menopausal age <36 years old group and menopausal age group 36 to 39 years old group was significantly higher than the control group shui (t = 2.283,2.165, P <0.05), 25-OH-VD levels were significantly lower than the control group (t = 8.340, 8.247, respectively, P <0.05). There was no significant difference in BGP levels between groups (F = 1.094, P> 0.05). BMD of lumbar spine was negatively correlated with PTH (R = -0.549, P <0.05), but positively correlated with 25-OH-VD (R = 0.507, P <0.05). Conclusions Osteoporosis in patients with premature ovarian failure is closely related to the increase of PTH level and the decrease of 25-OH-VD level.
