目的探讨骨科内置物术后感染病原菌检测结果及利福平联合万古霉素与左氧氟沙星对耐甲氧西林金黄色葡萄球菌(MRSA)、凝固酶阴性葡萄球菌(MRCNS)体外抗菌活性,为骨科内置物术后感染的治疗提供参考。方法对2010年1月-2015年7月63例医院行骨科术后发生内置物术后感染者局部分泌物行细菌培养及药敏试验,进行体外利福平联合万古霉素与左氧氟沙星对MRSA、MRCNS体外抗菌活性。结果 63例患者均送检感染局部分泌物,培养阳性率为84.13%,分离出55株病原菌,以铜绿假单胞菌为主,占29.09%;利福平联合万古霉素对MRSA、MRCNS体外抗菌活性86.67%为协同、13.33%为相加,未出现拮抗及无效。结论骨科内置物术后感染病原菌主要为铜绿假单胞菌,利福平联合万古霉素与左氧氟沙星对MRSA、MRCNS有较好的协同及相加体外抗菌活性。
Objective To investigate the detection results of pathogenic bacteria in the orthopedics and the antibacterial activity of rifampin combined with vancomycin and levofloxacin against methicillin-resistant Staphylococcus aureus (MRSA) and coagulase-negative staphylococci (MRCNS) Postoperative infection treatment for reference. Methods From January 2010 to July 2015, 63 cases of local secretions after orthopedic surgery were treated with antibiotics and drug susceptibility test in 63 hospitals. The effects of rifampicin combined with vancomycin and levofloxacin on MRSA, MRCNS antibacterial activity in vitro. Results The positive rate of infection was 84.13% in 63 patients, and 55 pathogens were isolated, mainly Pseudomonas aeruginosa, accounting for 29.09%. Rifampicin and vancomycin were effective against MRSA and MRCNS in vitro Antibacterial activity of 86.67% for the synergy, 13.33% for the addition, did not appear antagonistic and ineffective. Conclusion The pathogenic bacteria in the postoperative orthopedics are mainly Pseudomonas aeruginosa. The combination of rifampicin with vancomycin and levofloxacin has good synergistic effect on MRSA and MRCNS in vitro and in vitro antibacterial activity.
