目的在预防儿童第一恒磨牙龋病中使用两种不同窝沟封闭材料进行疗效对比。方法选择在郑州市口腔医院健康体检的6~9岁牙齿健康儿童50人,共82对第一恒磨牙,采用自身对照方法,右侧第一恒磨牙用窝沟封闭剂和涂氟并设为观察组,左侧第一恒磨牙用常规窝沟封闭,并设为对照组。封闭术后1年、2年、3年后复查,比较两组龋齿发生率以及窝沟封闭剂脱落率。结果对照组封闭术后1年发生龋齿4例(龋齿率4.9%),2年发生龋齿15例(龋齿率18.3%),3年发生龋齿16例(龋齿率19.5%);观察组封闭术后1年发生龋齿2例(龋齿率2.4%),2年发生龋齿4例(龋齿率4.9%),3年发生龋齿5例(龋齿率6.1%);两组封闭术后2年和3年的龋病发生率差异有统计学意义(2年χ~2=5.298,3年χ~2=4.325,P<0.05)。对照组封闭术后1年封闭剂脱落4例(脱落率4.9%),2年脱落7例(脱落率8.5%),3年脱落9例(脱落率11.0%);观察组封闭术后1年封闭剂脱落2例(脱落率2.4%),2年脱落4例(脱落率4.9%),3年脱落4例(脱落率4.9%);两组封闭术后2年和3年的封闭剂脱落率差异有统计学意义(2年χ~2=5.64,3年χ~2=4.45,P<0.05)。结论和常规窝沟封闭术相比,同时使用窝沟封闭剂和涂氟对预防儿童第一恒磨牙龋病具有更好的远期疗效,值得临床推广。
Objective To compare the efficacy of two different pit and fissure sealants in preventing the first permanent molar dental caries in children. Methods A total of 82 pairs of first permanent molars with 50 healthy children aged 6 ~ 9 years old undergoing physical examination in Dental Hospital of Zhengzhou City were enrolled in this study. The first permanent molars with pit and fissure sealant In the observation group, the first permanent molars on the left side were closed with conventional pit and fissure and set as the control group. One year, two years and three years after the operation were closed. The incidence of dental caries and the pit and fissure sealant shedding rate were compared between the two groups. Results In the control group, 4 cases (4.9%) had caries in 1 year after closure, 15 cases (18.3%) had caries in 2 years, 16 cases (19.5%) had caries in 3 years, Two cases of caries occurred in one year (2.4% of dental caries rate), four cases of caries in two years (4.9% of caries cases), five cases of caries in three years (caries rate of 6.1%), two years and three years The incidence of dental caries was significantly different (2 years χ ~ 2 = 5.298, 3 years χ ~ 2 = 4.325, P <0.05). In the control group, there were 4 cases (4.9%) of the blocking agent shedding one year after the operation was closed, 7 cases (shedding rate 8.5%) in 2 years and 9 cases (shedding rate 11.0%) in 3 years; 2 cases of abscission (shedding rate 2.4%), 2 cases of shedding in 4 cases (shedding rate of 4.9%), 3 cases of shedding in 4 cases (shedding rate of 4.9%); 2 and 3 years after the closure of the two groups of sealant shedding The difference was statistically significant (2 years χ ~ 2 = 5.46, 3 years χ ~ 2 = 4.45, P <0.05). Conclusion Compared with conventional pit and fissure sealant, the simultaneous use of pit and fissure sealant and fluoride-coated fluoride has a better long-term curative effect in preventing children’s first permanent molar decay and is worthy of clinical promotion.
