目的观察急性心肌梗死(AMI)患者入院时心率水平与住院期间心功能的关系。方法回顾性分析2005-2007年栾川县人民医院的ST段抬高AMI患者136例,根据入院时心率水平分为<80次/min组、80~100次/min组及>100次/min组,分别记录左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)及左室射血分数(LVEF)。结果三组患者年龄、性别、吸烟史、高血压、糖尿病史、血脂异常差异均无统计数意义(P>0.05)。心率>100次/min组患者LVEDD为(52.8±7.4)mm明显高于80~100次/min组[(51.8±6.2)mm,P<0.05]及<80次/min组[(50.2±5.4)mm,P<0.01];心率>100次/min组患者LVESD(40.3±8.6)mm也明显高于80~100次/min组[(38.5±6.3)mm,P<0.05]及<80次/min组[(34.6±5.5)mm,P<0.01];而心率>100次/min组患者LVEF(44.2%±10.6%)明显低于80~100次/min组[(48.5%±11.2%),P<0.05]及<80次/min组[(55.6%±10.3%),P<0.01],后两组之间差异也有统计数意义(P<0.05)。结论随心率增快,AMI患者心功能明显降低,对AMI患者应密切关注和控制心率,改善预后。
Objective To investigate the relationship between heart rate and hospitalized cardiac function in patients with acute myocardial infarction (AMI). Methods A retrospective analysis of 136 patients with ST-elevation AMI in Luanchuan People’s Hospital from 2005 to 2007 was performed. The patients were divided into two groups according to the heart rate at admission: 80 / min, 80-100 / min and> 100 / min The left ventricular end diastolic dimension (LVEDD), left ventricular end systolic dimension (LVESD) and left ventricular ejection fraction (LVEF) were recorded. Results There was no statistical significance in the differences of age, sex, smoking history, hypertension, history of diabetes and dyslipidemia among the three groups (P> 0.05). LVEDD was significantly higher in patients with heart rate> 100 beats / min (52.8 ± 7.4) mm than those in patients with 80-100 beats / min (51.8 ± 6.2) mm, P <0.05 and 50.2 ± 5.4 ), P <0.01]. LVESD (40.3 ± 8.6) mm in heart rate> 100 beats / min group was also significantly higher than that in 80 to 100 beats / min group (38.5 ± 6.3) mm, P <0.05 / (34.6 ± 5.5) mm, P <0.01], while LVEF was significantly lower in patients with heart rate> 100 beats / min (44.2% ± 10.6% vs 80 ~ 100 / ), P <0.05], and <80 / min [(55.6% ± 10.3%), P <0.01]. There was also statistical significance between the two groups (P <0.05). Conclusion With the rapid increase of heart rate, the cardiac function of patients with AMI is significantly reduced. The AMI patients should pay close attention to and control the heart rate and improve the prognosis.
