目的研究比例辅助通气(PAV)估算患者呼吸系统力学参数的有效率及PAV较其它通气模式可能具有的优势。方法以模拟肺作为实验平台,研究PAV模式下呼吸机对146例呼吸系统顺应性(C)和146例气阻(R)估算的有效率,以及206例不同参数的PAV与压力辅助通气(PSV)及双水平正压通气(Bi Level)在呼吸力学上的差别。结果相对于传统Younes方法,本研究对C估值的决定系数由0.820提高到0.943,R估值的决定系数由0.489提高到0.890;PAV较PSV和Bilevel在气道平均压和吸气时间上有显著优势。结论 PAV实现了实时的呼吸系统力学参数定量估计且利于肺保护通气。
Objective To investigate the effectiveness of proportional assisted ventilation (PAV) in estimating respiratory parameters and the possible advantages of PAV over other ventilation patterns. Methods Using simulated lung as experimental platform, the effectiveness of ventilator in 146 respiratory system compliance (C) and 146 cases of resistance (R) estimation in patients with PAV was studied, and 206 patients with different parameters of PAV and PSV ) And bi-level positive pressure ventilation (Bi Level) on the difference in respiration mechanics.
