目的:探讨血管内皮细胞生长因子(VEGF)及可溶性血管内皮细胞生长因子受体1(sFlt-1)在子痫前期患者胎盘组织中的表达。方法:采用免疫组化链霉菌抗生物素蛋白-过氧化物酶连接(SP)法对25例重度子痫前期患者(子痫前期组)和20例正常妊娠妇女(对照组)的胎盘组织中的VEGF、sFlt-1蛋白表达进行检测;同时应用RT-PCR技术检测两组孕妇胎盘组织中VEGF、sFlt-1 mRNA的水平。结果:①子痫前期组VEGF蛋白表达(+++)者4例(16%,4/25),明显低于对照组的9例(45%,9/20),差异有统计学意义(P<0.01);两组sFlt-1蛋白表达(+++)分别为13例(52%,13/25)、4例(20%,4/20),两组比较差异有统计学意义(P<0.01)。②子痫前期组sFlt-1的mRNA水平为0.876±0.039,明显高于对照组的0.557±0.209,差异有统计学意义(P<0.05);子痫前期组VEGF的mRNA水平略高于对照组,但差异无统计学意义(P>0.05)。结论:VEGF及其受体sFlt-1在子痫前期患者胎盘组织中的异常表达,可能参与妊娠早期子宫螺旋动脉重铸障碍,从而引起胎盘绒毛浅着床及胎盘缺血缺氧。
Objective: To investigate the expression of vascular endothelial growth factor (VEGF) and soluble vascular endothelial growth factor receptor 1 (sFlt-1) in placenta of preeclampsia. Methods: The placental tissues of 25 patients with severe preeclampsia (preeclampsia group) and 20 normal pregnant women (control group) were immunized with immunohistochemical streptavidin-peroxidase (SP) The levels of VEGF and sFlt-1 mRNA in the placenta were detected by RT-PCR. Results: ① The expression of VEGF protein in preeclampsia group was significantly higher than that in control group (4%, 16%, 4/25), which was significantly lower than that in control group (45%, 9/20) P <0.01). The expressions of sFlt-1 protein in two groups were 13 cases (52%, 13/25) and 4 cases (20%, 4/20) respectively. There was significant difference between the two groups P <0.01). ② The mRNA level of sFlt-1 in preeclampsia group was 0.876 ± 0.039, which was significantly higher than that in control group (0.557 ± 0.209, P <0.05). The mRNA level of VEGF in preeclampsia group was slightly higher than that in control group , But the difference was not statistically significant (P> 0.05). Conclusion: The abnormal expression of VEGF and its receptor sFlt-1 in placenta of preeclampsia patients may be involved in the remodeling disorder of uterine spiral artery in early pregnancy, resulting in placental villi implantation and placental ischemia and hypoxia.
