目的:探讨同型半胱氨酸(homocysteine,Hcy)与2型糖尿病(T2DM)大血管病变的关系。方法:取219例2型糖尿病患者,彩色多普勒超声检测颈动脉内膜中层厚度(intima-media thickness,IMT),根据IMT值分为血管正常组、内膜增厚组、血管斑块组,取健康人群50例为对照,比较各组间年龄、BMI、腰高比、Hcy、HbA1c,并行多元线性回归,观察上述指标对IMT的影响。结果:T2DM患者Hcy、HbA1c明显高于对照组,且Hcy、HbA1c随血管病变加重而增加。回归分析显示,Hcy、HbA1c升高是T2DM动脉硬化的独立危险因素。结论:除随访糖化血红蛋白外,早期检测Hcy水平或是及早发现糖尿病大血管并发症的有效手段。
Objective: To investigate the relationship between homocysteine (Hcy) and macroangiopathy in type 2 diabetes mellitus (T2DM). Methods: Totally 219 patients with type 2 diabetes mellitus were examined with color Doppler ultrasound intima-media thickness (IMT). According to IMT, they were divided into three groups: normal vessel group, intimal thickening group and vascular plaque group , Taking 50 healthy people as control group. The age, BMI, waist-height ratio, Hcy, HbA1c were compared among groups, and the multivariate linear regression was used to observe the influence of these indexes on IMT. Results: The Hcy and HbA1c in patients with T2DM were significantly higher than those in the control group, and Hcy and HbA1c increased with the increase of vascular lesions. Regression analysis showed that Hcy and HbA1c were independent risk factors of atherosclerosis in T2DM. Conclusion: In addition to follow-up of HbA1c, early detection of Hcy levels or early detection of diabetic macrovascular complications is an effective means.
