目的探讨社区老年高血压病患者阿司匹林不合理停药的原因及服药依从性的干预效果。方法将260例停服阿司匹林的老年高血压病患者,采用抽签法随机分为2组:干预组和对照组各130例。通过对社区医生及入选病人开展有关阿司匹林相关知识讲座宣传,并对干预组病人进行个体化指导及定期随访,观察两组病人干预当月及3个月后的服药改善情况。结果不合理停药原因,因害怕副作用自行停药的占78.46%(204/260)。经过培训后,两组患者在第一个月服药的人数没有显著差异(P>0.05),但三个月后比较两组有显著性差异(P<0.05)。结论有效地阿司匹林知识培训及个体化指导可以减少老年高血压病患者阿司匹林的不合理停药和提高服药依从性。
Objective To investigate the reasons for the unreasonable discontinuation of aspirin in community elderly hypertensive patients and the effect of medication adherence. Methods A total of 260 elderly patients with aspirin taking aspirin were randomized into two groups by random selection: 130 in intervention group and control group. Publicity of aspirin-related knowledge was carried out for community doctors and selected patients. Individualized interventions and regular follow-up were conducted among patients in the intervention group to observe the medication improvement in the two months and three months after intervention. The result was unreasonable withdrawal reasons, self-medication for fear of side effects accounted for 78.46% (204/260). After training, there was no significant difference between the two groups in the first month (P> 0.05), but there was a significant difference between the two groups after three months (P <0.05). Conclusions Effective aspirin knowledge training and individualized instruction can reduce the unreasonable discontinuation of aspirin in elderly hypertensive patients and improve medication adherence.
