目的对热-染料双指示剂稀释法和血管外肺水微机测量系统(EVLW)检测的血管外肺热容量(ETV)进行可靠性验证和评价。方法34只犬分为正常对照组与肺水肿实验组。将ETV与排除肺残余血量测得的血管外肺组织(ELM)和血管外肺水(ELW)比较。结果ETV与ELW相关系数r=0.9509,ETV与ELM相关系数r=0.9546,ETV重复测量值之间r=0.9040,变异系数CV=5.68%,对照组ETV最后/ELM为1.0094±0.033,实验组为0.9401±0.091。结论本系统能较准确、特异地对血管外肺水做定量分析,重复性好,可实时动态观察肺水的变化,从而早期诊断肺水肿。ETV对正常肺水略高估,而在严重肺水肿时略低估。
Objective To verify and evaluate the reliability of thermal-dye double indicator dilution method and extravascular lung heat capacity (ETV) measured by extravascular lung water micro-computer system (EVLW). Methods Thirty-four dogs were divided into normal control group and pulmonary edema group. ETV was compared with extravascular lung tissue (ELM) and extravascular lung water (ELW) measured by exclusion of residual lung volume. Results The correlation coefficient of ETV and ELW was r=0.9509, the correlation coefficient of ETV and ELM was r=0.9546, the value of repeated measurement of ETV was r=0.9040, the coefficient of variation was CV=5.68%, and the control group was ETV last/ELM. It was 1.0094±0.033 and the experimental group was 0.9401±0.091. Conclusion The system can quantitatively analyze the extravascular lung water accurately and specifically. The reproducibility is good. The changes of the lung water can be dynamically observed in real time and the pulmonary edema can be diagnosed early. ETV is slightly overestimated in normal lung water, and slightly underestimated in severe pulmonary edema.
