羟基喜树碱联合顺铂与氟尿嘧啶和亚叶酸钙治疗晚期直肠癌的疗效分析及文献复习

目的分析羟基喜树碱联合顺铂与氟尿嘧啶和亚叶酸钙治疗晚期直肠癌的临床疗效。方法 2例晚期直肠癌患者,按以下方式用药:羟喜树碱10 mg/d,静脉滴注4 h;顺铂10 mg/d;氟尿嘧啶每天注射500~750 mg/m2,并在使用氟尿嘧啶前使用亚叶酸钙200 mg/d。联合用药5 d,每3周为1个疗程。在2个疗程后,根据WHO1981版有关实体瘤治疗疗效的评价标准,WHO有关抗癌药物毒副作用的分级标准,进行临床疗效评估。结果 2例患者均达CR(完全缓解)。并发症情况:出现恶心、食欲减退,3级骨髓抑制,未见明显肝肾功能损伤。结论羟基喜树碱联合顺铂与氟尿嘧啶和亚叶酸钙治疗晚期直肠癌,疗效显著,副作用小,值得推广。 Objective To analyze the clinical efficacy of hydroxycamptothecin combined with cisplatin and fluorouracil and leucovorin in the treatment of advanced rectal cancer. Methods Two patients with advanced colorectal cancer were treated with 10 mg / d of hydroxycamptothecin, 4 h of IV infusion, 10 mg / d of cisplatin, and 500-750 mg / m2 of fluorouracil daily before fluorouracil Use leucovorin 200 mg / d. Combination therapy 5 d, every 3 weeks for a course of treatment. After 2 courses of treatment, according to WHO1981 version of the evaluation criteria for the curative effect of solid tumors, the WHO classification standard on the side effects of anticancer drugs for clinical efficacy evaluation. Results Both patients achieved CR (complete remission). Complications: nausea, loss of appetite, grade 3 bone marrow suppression, no significant liver and kidney damage. Conclusions Hydroxycamptothecin combined with cisplatin and fluorouracil and leucovorin in the treatment of advanced rectal cancer have significant curative effect and small side effects and are worthy of popularization.
