目的分析无锡市儿童的死亡现状和流行特征,为开展防制工作提供科学依据。方法对无锡市2008—2015年儿童死亡资料进行分析,计算死亡率、死亡率的年度变化百分比(APC)和潜在寿命损失年数。结果 2008—2015年儿童平均死亡率为37.89/10万(标化死亡率为40.43/10万),男童明显高于女童(u=7.315,P<0.01);8年间死亡率无明显变化趋势(APC=1.01%,95%CI=-4.76%~7.11%,P>0.05),但儿童死亡在全人群死亡中的构成比下降了40.40百分点。前3位主要死因为损伤和中毒、起源于围生期的某些情况、先天畸形变性和染色体异常,分别占27.84%,21.03%,20.75%;0~<1岁组的主要死因为起源于围生期的某些情况,1~17岁各年龄组主要死因为损伤和中毒。减寿总年数为67 368.5人年,减寿率为12.63‰,平均减寿年数为61.41年;减寿前3位主要死因为损伤和中毒、肿瘤、先天畸形变性和染色体异常。结论损伤和中毒、起源于围生期的某些情况、先天畸形变性和染色体异常是导致无锡市儿童死亡的主要原因。应采取综合性防制措施降低死亡率。
Objective To analyze the current situation and prevalence of death among children in Wuxi and provide a scientific basis for prevention and control work. Methods Data of child deaths in Wuxi from 2008 to 2015 were analyzed to calculate the annual death rate, annual change percentage (APC) and years of potential life loss. Results The average death rate of children in 2008-2015 was 37.89 / lakh (standardized rate was 40.43 / lakh), while that of boys was significantly higher than that of girls (u = 7.315, P <0.01). There was no significant change in mortality rate (APC = 1.01%, 95% CI = -4.76% -7.11%, P> 0.05). However, the proportion of child deaths among the total population deaths decreased by 40.40 percentage points. The first three major causes of death were injury and poisoning, with some origins in the perinatal period, congenital deformity and chromosomal abnormalities, accounting for 27.84%, 21.03% and 20.75% respectively. The main causes of death in 0 ~ <1 year old group originated in In some cases of the perinatal period, the main causes of death in all age groups between the ages of 1 and 17 are injury and poisoning. The total number of years of life expectancy reduction was 67 368.5 person-years, the rate of reduction in life was 12.63 ‰, and the average number of years of life lost was 61.41 years. The three leading causes of death were injury and poisoning, tumor, congenital deformity and chromosomal abnormalities. Conclusions Injury and poisoning originated in some cases during the perinatal period. Congenital deformity and chromosomal abnormalities are the major causes of child death in Wuxi City. Should take comprehensive prevention and control measures to reduce mortality.
