目的评价常规超声(US)、超声造影(CEUS)及2者联合应用在小乳腺癌诊断中的效果。方法分别采用US、CEUS对85例患者的乳腺小肿块(病灶直径≤2 cm)进行检查,分析病灶影像资料,以病理诊断为金标准,比较US、CEUS及联合应用的诊断价值,并绘制受试者工作特征曲线(ROC)。结果经手术切除的85例患者乳腺病灶中,恶性49例,占57.65%,良性36例,占42.35%;联合应用组各项指标均优于US及CEUS单独组;US、CEUS单独组及联合应用组的ROC曲线下面积(AUC)分别为0.712、0.755、0.820,经Z检验,联合应用2种检测方式的AUC大于单独US组(P=0.028)和CEUS组(P=0.097)。结论 US与CEUS联合应用、综合判断,可以提高小乳腺癌诊断水平,具有重要的临床应用价值。
Objective To evaluate the effect of conventional ultrasound (US), contrast-enhanced ultrasonography (CEUS) and their combination in the diagnosis of small breast cancer. Methods Totally 85 small breast masses (diameter less than or equal to 2 cm) were examined by US and CEUS respectively. The lesion images were analyzed and the diagnostic value of pathology was taken as the gold standard. The diagnostic value of US, CEUS and combined application were compared. Tester working characteristic curve (ROC). Results Among the 85 patients who underwent surgical resection, 49 were malignant, accounting for 57.65%, and 36 were benign, accounting for 42.35%. The indexes of combination group were superior to those of US and CEUS alone group. The area under the ROC curve (AUC) of the application group was 0.712, 0.755 and 0.820, respectively. The AUC of the two detection methods by Z test was higher than that of the US group alone (P = 0.028) and CEUS group (P = 0.097). Conclusion US and CEUS combined application, comprehensive judgment, can improve the diagnosis of small breast cancer, has important clinical value.
