目的观察肺复张对体外循环先天性心脏病(先心病)术后早期急性呼吸窘迫综合征(ARDS)患儿的疗效及不良反应。方法 20例先心病术后ARDS患儿,均给予肺复张治疗。观察患儿氧合指数(Pa O_2/Fi O_2)及血流动力学变化。结果复张中、复张后Pa O_2/Fi O_2均优于复张前(P<0.05);复张中心率(HR)、平均动脉压(MAP)、中心静脉压(CVP)与其他时间点比较,差异具有统计学意义(P<0.05)。全部病例未发生气压伤或其他并发症。结论对先心病术后早期发生ARDS患儿进行肺复张,可有效改善肺氧合功能,对血流动力学影响的可控性强。
Objective To observe the efficacy and adverse reactions of pulmonary reperfusion on children with acute respiratory distress syndrome (ARDS) after cardiopulmonary bypass (CHD). Methods Twenty patients with ARDS after congenital heart disease were given pulmonary reexpansion. Oxygenation index (Pa O 2 / Fi O 2) and hemodynamic changes in children were observed. Results During reexpansion, Pa O 2 / Fi O 2 was better than pre-reexplosion (P <0.05) after reexpansion; heart rate (HR), mean arterial pressure (MAP), central venous pressure (CVP) and other time points The difference was statistically significant (P <0.05). No barotrauma or other complications occurred in all cases. CONCLUSIONS: Pulmonary reexpansion of children with ARDS early after congenital heart disease can effectively improve pulmonary oxygenation function and has strong controllability on hemodynamics.
