心脏对持久性动脉压升高的适应作用是引起左室肥大。因而,普遍认为左室肥大是对后负荷增加的一种有益的生理性代偿过程。然而,Framingham的研究证明,心电图呈左室肥大者,猝死和急性心肌梗塞的危险性大于正常心脏者。作者分析了3组病人:血压正常者14例(组Ⅰ);无并发症的原发性高血压10例(组Ⅱ);高血压伴心电图左室肥大16例(组Ⅲ)。组Ⅲ在M型超声心动图上左室后壁厚度>1.1(平均1.34±0.17)cm,按Bennett和Evans公式计算的左室重量平均为323±
Cardiac adaptation to persistent elevated arterial pressure is caused by left ventricular hypertrophy. Therefore, it is generally accepted that left ventricular hypertrophy is a beneficial physiological compensatory process for post-load increase. However, Framingham’s study showed that electrocardiogram showed left ventricular hypertrophy, sudden death and acute myocardial infarction risk than normal heart. The authors analyzed three groups of patients: normotensive in 14 patients (group Ⅰ); no complications of essential hypertension in 10 patients (group Ⅱ); hypertension with ECG left ventricular hypertrophy in 16 patients (group Ⅲ).
