目的:对口腔粘膜损害缘分离的上皮组织作直接免疫荧光检查( D I F) ,探讨应用该技术诊断天疱疮的可靠性。方法:选择22 例口腔寻常型天疱疮,从患者口腔粘膜损害缘分离上皮,制成冰冻切片作为 D I F 的底物,用异硫氰酸荧光素( F I T C)标记的兔抗人抗体染色检查,并与同一患者经传统活检取材的粘膜组织( 含部分粘膜下层的完整粘膜块) 的 D I F 检查作比较。结果:分离上皮的 D I F,22 例全部有 Ig G 沉积在棘细胞间,并有3 例 Ig A、、1 例 Ig M、13 例 C3 沉积在棘细胞间,与传统活检法的粘膜组织 D I F检查结果相同。结论:从口腔粘膜损害缘分离的上皮组织作 D I F检查,用于诊断天疱疮其结果可靠,操作简便易行,创伤轻微。
OBJECTIVE: To determine the direct immunofluorescence (D I F) of epithelial tissue isolated from the mucosal lesions of the oral mucosa and to evaluate the reliability of the technique in the diagnosis of pemphigus. Methods: Twenty-two cases of pemphigus vulgaris were selected and epithelial cells were isolated from the mucosal lesion edge of the patient. Frozen sections were made as the substrate of D I F. Rabbit anti-FITC labeled Human antibody staining was compared with that of the same patient with conventional biopsy of mucosal tissue (with intact mucosal masses of partial submucosa). RESULTS: All of the 22 cases with IgF deposited in the epithelium had IgG deposited in the spinous cells with 3 cases of IgA, 1 case of IgM and 13 cases of C3 deposited in the spinous cells. Compared with the conventional biopsy mucosa D I F same test result. Conclusion: The epithelial tissue isolated from the mucosal lesion edge of the oral mucosa for the D I F examination for the diagnosis of pemphigus its reliable results, easy to operate, minor trauma.
