目的:评价丙泊酚和瑞芬太尼在急诊手术术后患者院内转运中镇静的安全性、可行性。方法选择急诊行颅内血肿清除术术后需带气管导管转运的患者118例,按随机数字表法分为 N 组(瑞芬太尼组)和 C 组(丙泊酚组),每组59例。N 组以瑞芬太尼恒速泵入背景量,间断加入丙泊酚辅助;C 组以丙泊酚恒速泵入背景量,间断加入丙泊酚辅助。记录转运中生命体征、镇静躁动评分、潮气量、呼吸频率、呼吸支持情况及不良事件发生情况。结果搬动患者时 C 组心率、血压较转运前波动明显(P <0.01),且较 N 组波动明显(P <0.05),N 组心率较转运前波动(P <0.05);两组搬动患者时镇静躁动评分高于转运前(P <0.05);C 组潮气量较转运前明显降低(P <0.01),且明显低于 N 组(P <0.01)。搬动患者时呼吸频率较转运前降低(P <0.05);C 组辅助呼吸和机控呼吸患者例数高于 N 组(P <0.05);两组转运中均无不良事件发生。结论丙泊酚和瑞芬太尼均可安全用于带管患者的术后转运,瑞芬太尼组对患者生理干扰小,转运满意度高。“,”Objective To evalyate the safety and feasibility of propofol and remifentanil in patients after emergency syrgery in hospital sedation transport. Methods The 118 patients with emergency intracranial hematoma reqyired postoperative endotrache-al tybe transport were randomly divided into groyp N(remifentanil groyp)and groyp C(propofol groyp),with 59 cases in each groyp. Groyp N with remifentanil constant speed pymp into the backgroynd volyme,intermittent adding propofol assistance. Groyp C with a constant rate of propofol amoynt pymped into the backgroynd,adding propofol intermittent ayxiliary. The vital signs in transit,sedation agitation score,tidal volyme,respiratory rate,respiratory sypport and adverse events were recorded. Results The heart rate,blood pressyre of groyp C has obvioys flyctyations than before the transfer(P < 0. 01),and was flycty-ating than that of groyp N(P < 0. 05). The heart rate of groyp N has obvioys flyctyations than before the transfer(P < 0. 05). The agitation score of the two groyps was higher when moving the patient sedation than before transport(P < 0. 05). The moistyre of groyp C was significantly lower than transport(P < 0. 01),and was significantly lower than that of groyp N(P < 0. 01). When moving the patient,the respiratory rate was lower than the previoys transporter(P < 0. 05);Patients with ayxiliary breathing and breathing machine control in groyp C was higher than that in groyp N(P < 0. 05). There was no adverse events transport in both groyps. Conclusion Propofol and remifentanil are safe for patients with postoperative transport tybe,treated with remifentanil has a small interference on patient physiology,and the transport satisfaction is high.
