目的探讨乙醛脱氢酶1(ALDH1)在肿瘤细胞质中表达和在肿瘤细胞基质中表达与乳腺癌患者预后的关系。方法选取该院乳腺外科接受手术的乳腺癌患者为研究对象,应用免疫组织化学方法检测160例乳腺癌患者的病理标本,研究ALDH1在肿瘤细胞质中表达和肿瘤基质中表达与患者年龄、月经史、肿瘤大小、TNM分期、淋巴结转移数目、ER、PR、HER-2、Ki-67等指标及临床预后的关系。结果 ALDH1在肿瘤细胞质中表达56例,占35%,在肿瘤细胞基质中表达64例,占40%。ALDH1在肿瘤基质中表达与患者年龄>50岁及ER表达情况有统计学意义(P=0.013;P=0.022)。在乳腺癌细胞质中ALDH1阳性表达较ALDH1阴性表达患者的无病生存期(RFS)和总生存期(OS)明显缩短,差异有统计学意义(P<0.05)。在乳腺癌基质中ALDH1阳性与乳腺癌患者预后生存时间差异均无统计学意义(均P>0.05)。结论在乳腺癌细胞质中ALDH1阳性表达较ALDH1阴性表达患者的RFS和OS明显缩短;在乳腺癌基质中ALDH1阳性表达与乳腺癌患者年龄及ER表达情况具有相关性,与其他临床参数无相关性;ALDH1在肿瘤基质中阳性表达对评价乳腺癌患者预后生存没有价值。
Objective To investigate the relationship between the expression of aldehyde dehydrogenase 1 (ALDH1) in the tumor cytoplasm and the expression of ALDH1 in the tumor cell matrix and the prognosis of patients with breast cancer. Methods A total of 160 cases of breast cancer were examined by immunohistochemistry. The expression of ALDH1 in the tumor cytoplasm and the expression of ALDH1 in the tumor stroma were compared with the age, history of menstruation, Tumor size, TNM stage, the number of lymph node metastasis, ER, PR, HER-2, Ki-67 and other indicators and clinical prognosis. Results The expression of ALDH1 in the cytoplasm of the tumor was 56 (35%) and 64 (40%) in the tumor cell matrix. The expression of ALDH1 in the tumor stroma was associated with a statistically significant (P = 0.013; P = 0.022) correlation with age of patients> 50 years and ER expression. The ALDH1 positive expression in breast cancer cytoplasm was significantly shorter than that in patients with negative ALDH1 expression (RFS) and overall survival (OS). The difference was statistically significant (P <0.05). There was no significant difference in the survival time between ALDH1 positive breast cancer and breast cancer (all P> 0.05). Conclusion The positive expression of ALDH1 in breast cancer cytoplasm was significantly shorter than that in patients with negative ALDH1 expression. The positive expression of ALDH1 in breast cancer was correlated with age and ER expression, but not with other clinical parameters. The positive expression of ALDH1 in tumor stroma has no value in evaluating the prognosis of patients with breast cancer.
