目的:探讨和研究首次透析负责制管理模式应用于血液净化中心对医疗质量的改善效果。方法:浙江丽水市中心医院在2013年开始实施首次透析负责制管理,以2013年全年的103例血液透析患者为研究对象,并选取浙江丽水市中心医院2012年的92例血液透析患者作为对照,采用明尼苏达满意问卷进行调查,让患者对护理人员的工作满意度进行评价,并对患者的透析充分性进行评价。结果:在进行透析充分性评价的5个项目中,2013年患者的血压、血清白蛋白、蛋白分解率以及尿素清除指数要显著优于2012年,差异具有统计学意义(P<0.05);患者满意度对比2013年为98.9%,显著优于2012年的93.5%,差异具有统计学意义(P<0.05)。结论:在血液净化中心实施首次透析负责制管理模式能够有效的提高护理效果,对患者的透析充分性有着明显的提高,同时也提高了患者的护理满意度。
Objective: To investigate and study the first dialysis responsibility system management model applied to the blood purification center to improve the quality of medical care. Methods: Lishui Central Hospital implemented the first management of dialysis responsibility system in 2013, and took 103 hemodialysis patients in 2013 as the research object, and selected 92 cases of hemodialysis patients from Lishui Central Hospital in 2012 as control , A Minnesota Satisfaction Questionnaire survey was conducted to allow patients to evaluate the job satisfaction of nursing staff and evaluate the patient’s adequacy of dialysis. Results: Among 5 items of dialysis adequacy evaluation, the patients’ blood pressure, serum albumin, protein decomposition rate and urea clearance index in 2013 were significantly better than those in 2012 (P <0.05). Patients Satisfaction was 98.9% in 2013, significantly better than 93.5% in 2012, the difference was statistically significant (P <0.05). Conclusion: The implementation of the first dialysis accountability management model in the blood purification center can effectively improve the nursing effect, significantly improve the adequacy of dialysis patients, and improve patient satisfaction.
