目的分析不同胎龄新生儿体表胃电图的临床特点。方法收集不同胎龄新生儿的体表胃电图资料。将收入我科不同胎龄的新生儿分为4组:Ⅰ组为胎龄≥37周;Ⅱ组为胎龄34~36+6周;Ⅲ组为胎龄32~33+6周;Ⅳ组为胎龄<32周,在生后1周内进行体表胃电图检测,记录并分析胃电图参数。应用ANOVA方法对各组间的差异进行统计学分析。对其中部分早产儿在生后1周内、2、4周分别进行胃电图检测,应用配对t检验检测同一病例的胃电图参数的前后差异。结果对141例新生儿进行了胃电图检测,Ⅰ组25例;Ⅱ44例;Ⅲ组33例;Ⅳ组39例。与成人和儿童相比所有新生儿的正常胃电慢波百分比均较低,平均为30.6%,而胃电节律紊乱百分比较高。各组间正常胃电慢波百分比、胃电节律过缓百分比及胃电节律过速百分比差异无统计学意义。24例早产儿生后4周及生后2周的胃电图参数与生后1周内相比差异无统计学意义。结论与成人和儿童相比,新生儿的胃肌电活动不成熟,生后1周内不同胎龄的早产儿与足月儿的胃电形式比较,差异无统计学意义,新生儿的胃肌电活动在生后4周时仍处于发育成熟阶段。
Objective To analyze the clinical features of body surface electrogastrogram of neonates with different gestational age. Methods The data of body surface electrogastrogram of neonates with different gestational age were collected. The newborns with different gestational ages in our department were divided into 4 groups: group Ⅰ was gestational age≥37 weeks; group Ⅱ was gestational age 34-36 + 6 weeks; group Ⅲ was gestational age 32-33 + 6 weeks; For gestational age <32 weeks, 1 week after birth surface electrogastrogram detection, recording and analysis of EGG parameters. The ANOVA method was used to analyze the differences among the groups. Some of them preterm infants within 1 week after birth, 2,4 weeks, respectively, the electrogastrogram were tested using the paired t test to detect the same case of EGG parameters before and after the difference. Results 141 cases of newborns were detected by electrogastrogram, Ⅰ group of 25 cases; Ⅱ 44 cases; Ⅲ group of 33 cases; Ⅳ group of 39 cases. Compared with adults and children, all newborns had a lower percentage of normal gastric slow waves with an average of 30.6%, while the percentage of gastric electrical rhythm disorders was high. There were no significant differences in the percentage of normal gastric electrical slow wave, gastric electrical rhythm bradycardia and gastric electrical rhythm tachycardia in each group. 24 cases of premature children after 4 weeks of life and 2 weeks after the EGG parameters compared with 1 week after birth, no significant difference. Conclusion Compared with adults and children, neonatal gastric myoelectric activity is immature, within 1 week after birth, gestational age in different gestational age preterm children and full-term children’s stomach form, the difference was not statistically significant, the neonatal gastric muscle Electrical activity is still in its mature stage at 4 weeks after birth.
