目的：对幽门螺杆菌（Ｈｐ）相关性萎缩性胃炎分型、肠化分型和不典型增生进行背景调查，探讨Ｈｐ感染在萎缩性胃炎发展中的作用。方法：１５３例萎缩性胃炎作出Ａ型、Ｂ型及ＡＢ型分类。用Ｗａｒｔｈｉｎ－Ｓｔａｒｒｙ染色判定Ｈｐ。组化阿尔辛蓝过碘酸雪夫反应ＡｌｃｉａｎＢｌｕｅ（ｐＨ２．５）／ＰＡＳ以及高碘酸－硼氢化钠－氢氧化钾－过碘酸雪夫反应ＰＢ／ＫＯＨ／ＰＡＳ做出小肠型肠化和结肠型肠化。病理诊断萎缩性胃炎、不典型增生及胃癌。结果：萎缩性胃炎Ａ型、Ｂ型及ＡＢ型三者之间Ｈｐ感染率存在显著差异（２５％、６２．２％、７８．４％）。Ｈｐ（＋）结肠型肠化发生率在Ｂ型与ＡＢ型之间存在显著差异（２１．４％、６３．６％）。Ｈｐ（＋）不典型增生在各型之间发生率无显著差异。随访Ｈｐ（＋）萎缩性胃炎癌发率在Ａ、Ｂ及ＡＢ型之间无显著性差异（０／１０、２／２７、５／２４）。结论：萎缩性胃炎ＡＢ型与Ｈｐ感染密切相关。Ｈｐ相关性萎缩性胃炎胃癌发生率各型之间无显著差异。Ｈｐ感染在慢性萎缩性胃炎发展中各型间感染率的差异与胃癌发生率不平行。提示发生胃癌的因素是综合作用或他因为主的结果
Objective: To investigate the relationship between Helicobacter pylori (Hp) -associated atrophic gastritis, intestinal metaplasia and dysplasia and to explore the role of Hp infection in the development of atrophic gastritis. Methods: Atrophic gastritis was classified into type A, type B and type AB by 153 cases. Hp was determined by Warthin-Starry staining. Histochemistries of Alcian Blue (pH 2.5) / PAS and periodic acid-sodium borohydride-potassium hydroxide-periodic acid Schiff reaction PB / KOH / PAS were used to make small intestine intestinal metaplasia and colon type Intestinal. Pathological diagnosis of atrophic gastritis, dysplasia and gastric cancer. Results: The prevalence of Hp infection in atrophic gastritis type A, B and AB was significantly different (25%, 62.2%, 78.4%). The incidence of Hp (+) colon-type intestinal metaplasia was significantly different between type B and type AB (21.4%, 63.6%). There was no significant difference in the incidence of Hp (+) dysplasia between the various types. Follow-up Hp (+) atrophic gastritis with no significant difference between the A, B and AB types (0/10, 2/27, 5/24). Conclusion: Atrophic gastritis type AB and Hp infection are closely related. Hp-related atrophic gastritis gastric cancer incidence between the various types of no significant difference. Hp infection in the development of chronic atrophic gastritis between the various types of infection rates and gastric cancer incidence is not parallel.
