为探讨慢性阻塞性肺病(COPD)合并急性呼吸功能衰竭的机械通气治疗。对近年ICU收治的14例COPD合并急性呼吸功能衰竭的患者使用机械通气治疗情况进行分析。14例病人经建立人工气道行机械通气治疗,有效控制肺部感染,加强重要脏器功能监测和支持。结果14例患者低氧和高碳酸血症明显改善。7例在急性呼吸衰竭早期即行机械通气治疗的病人,带机3至7天后顺利脱机,其余7例已经出现多器官损害的病人带机时间延长,最终死于多器官功能衰竭。结论:COPD合并急性呼吸功能衰竭的患者,经保守疗法,低氧及高碳酸血症无法纠正,应及时行机械通气治疗,改善氧供,纠正高碳酸血症,防止多器官损害加剧,才能提高救治成功率。
To investigate the mechanical ventilation in patients with chronic obstructive pulmonary disease (COPD) complicated with acute respiratory failure. In recent years, ICU admitted 14 cases of COPD patients with acute respiratory failure using mechanical ventilation treatment analysis. Fourteen patients underwent artificial airway mechanical ventilation to effectively control pulmonary infection and to strengthen the monitoring and support of vital organs. Results 14 cases of patients with hypoxemia and hypercapnia significantly improved. Seven patients who were treated with mechanical ventilation in the early stages of acute respiratory failure took the machine offline successfully 3 to 7 days after operation. The remaining 7 patients who had had multiple organ damage were lengthened and finally died of multiple organ failure. Conclusion: Patients with COPD complicated with acute respiratory failure can not be corrected by conservative therapy, hypoxemia and hypercapnia. Mechanical ventilation should be performed in time to improve oxygen supply, correct hypercapnia and prevent multiple organ damage. Treatment success rate.
