Since the nightmare of Bhopal in 1984, the United States has had many positive changes in chemical process safety. However, accidents involving active chemicals still occur frequently, and often lead to tragic results. This article examines four major active chemical accidents in the United States and reveals the consequences of uncontrolled chemical reactions. At midnight on December 21, 1984, one of the worst industrial accidents in history occurred in a U.S. pesticide production plant in Bhopal, India. It was an active chemical accident and methyl isocyanate reacted violently with water and caused a leak
