经腹壁下动脉置管采用吉西他滨联合顺铂方案化疗治疗复发后进展性膀胱癌

目的:探讨经腹壁下动脉置管采用吉西他滨联合顺铂(GC)方案动脉灌注化疗治疗复发后进展性膀胱癌的疗效。方法:对21例1年内复发的进展性膀胱尿路上皮癌患者术后行腹壁下动脉置管术并采用GC方案动脉灌注化疗3个疗程,定期膀胱镜检查,进行随访。结果:21例患者化疗后随诊18~24个月。肿瘤复发1例,复发率4.76%,不良反应小。对照组病例40例,术后随访24个月,复发15例,复发率为37.5%。根据连续校正的卡方检验,GC方案动脉灌注化疗组显著低于对照组的复发率,差异有统计学意义(P=0.032)。结论:经腹壁下动脉置管采用GC方案,动脉灌注化疗治疗复发后进展性膀胱癌患者疗效满意,不良反应小,耐受性好,值得临床推广采用。 Objective: To investigate the curative effect of transcatheter arterial infusion chemotherapy with gemcitabine combined with cisplatin (GC) in the treatment of recurrent bladder cancer after transabdominal arterial catheterization. Methods: Twenty-one patients with advanced bladder urothelial carcinoma who relapsed within one year underwent abdominal arterial catheterization and 3 courses of arterial infusion chemotherapy with GC regimen. The patients were followed up regularly by cystoscopy. Results: Twenty-one patients were followed up for 18-24 months after chemotherapy. Tumor recurrence in 1 case, the recurrence rate of 4.76%, adverse reactions. Control group of 40 patients, followed up for 24 months, 15 cases of recurrence, the recurrence rate was 37.5%. According to the continuous correction of Chi-square test, GC regimen arterial infusion chemotherapy group was significantly lower than the control group recurrence rate, the difference was statistically significant (P = 0.032). Conclusion: The inferior abdominal artery catheterization using GC program, arterial infusion chemotherapy for patients with recurrent bladder cancer after curative effect is satisfactory, the adverse reaction is small, well tolerated, worthy of clinical promotion.
