目的:通过测定子宫内膜异位症(EMs)患者血清中骨桥蛋白(OPN)的含量,探讨OPN在EMs发病机制中的作用及血清OPN对EMs诊断、鉴别诊断的价值。方法:采用ELISA法检测30例卵巢EMs患者及30例非EMs的卵巢良性肿瘤患者术前、术后血清中OPN的含量。结果:①研究组术前血清OPN水平为(20.80±8.79)ng/ml,高于对照组及术后(P<0.01);对照组术前、术后血清OPN水平差异无统计学意义(P>0.05)。②R-AFS期别增高,血清OPN水平也增高,Ⅲ期较Ⅱ期、Ⅳ期较Ⅲ期EMs患者血清OPN水平明显升高〔(18.89±3.75)ng/ml vs(11.55±2.23)ng/ml,P<0.05,(28.78±8.44)ng/ml vs(18.89±3.75)ng/ml,P<0.01〕。血清OPN水平与R-AFS分期间有相关性(r=0.771,P<0.01)。结论:血清OPN水平在EMs辅助诊断中具有一定的应用价值,监测OPN的动态变化可能有利于判定EMs的手术疗效及预后。血清OPN水平与R-AFS评分之间有一定的相关关系,可能与疾病的严重程度有关,亦可能参与了EMs的发生、发展过程。
Objective: To investigate the role of OPN in the pathogenesis of EMs and the value of serum OPN in the diagnosis and differential diagnosis of EMs by measuring the content of osteopontin (OPN) in serum of patients with endometriosis (EMs). Methods: The serum levels of OPN in 30 cases of ovarian EMs and 30 cases of non-ovarian benign ovarian tumor were detected by ELISA. Results: ① The preoperative serum OPN level in the study group was (20.80 ± 8.79) ng / ml, higher than that in the control group and postoperatively (P <0.01). There was no significant difference in OPN levels before and after operation in the control group > 0.05). ② The serum levels of OPN in stage Ⅲ were significantly higher than those in stage Ⅱ and Ⅳ than those in stage Ⅲ [(18.89 ± 3.75) ng / ml vs (11.55 ± 2.23) ng / ml , P <0.05, (28.78 ± 8.44) ng / ml vs (18.89 ± 3.75) ng / ml, P <0.01〕. There was a correlation between serum OPN level and R-AFS (r = 0.771, P <0.01). Conclusion: The level of serum OPN has some value in the auxiliary diagnosis of EMs. Monitoring the dynamic changes of OPN may be helpful to determine the surgical efficacy and prognosis of EMs. The correlation between serum OPN level and R-AFS score may be related to the severity of the disease and may also be involved in the occurrence and development of EMs.
