目的:本研究目的在于开发一种交互方式计算牙根方向,最终为种植外科种植体的植入方向、角度的把握及手术导板的制作提供科学依据。方法:(1)利用等值面提取的方式生成颌骨三维模型及实现牙根形态可视化,将牙根与周围骨组织进行分离。(2)在二维CT图像上选择合适剖面切片令其完整显示牙弓形态,在每一个牙根位置交互式放置和调整牙根轴向虚拟切割平面。(3)获取牙根轴位方向,借此评估缺失牙的原牙轴方向。结果:运用计算机辅助种植规划系统软件交互式调整,在矢状面、冠状面、轴状面及任意方向角度上观察分析拟种植术区骨结构。讨论:修复引导种植的目的在于恢复患者良好的咀嚼功能,种植体方向需保证一定的倾斜度,遵循Spee’s曲线或补偿曲线。基于上述原理可通过软件交互式分析方法,将模拟种植体调至与天然牙体长轴真正一致的位置。结论:应用颌骨CT三维影像牙轴断层交互设计分析技术,可以更全面地观察术区骨组织结构,制定科学的种植手术计划,通过手术导板确保种植手术的安全性与精确性。
OBJECTIVE: The purpose of this study is to develop an interactive way to calculate the root direction, and finally to provide a scientific basis for the implanting direction and angle of implanting surgical implant and the manufacture of surgical guide plate. Methods: (1) Using the method of isosurface extraction to generate the three-dimensional model of the mandible and the visualization of the root morphology, the root and the surrounding bone tissue were separated. (2) Select the appropriate section slice on the two-dimensional CT image to show the shape of the arch completely, and place and adjust the root axial virtual cutting plane interactively in each root position. (3) Obtain the direction of the root axis of the tooth root to evaluate the original tooth axis direction of the missing tooth. Results: Using computer-aided planting planning system software interactive adjustment, in the sagittal plane, coronal plane, axial plane and the direction of any direction to observe the anatomic bone structure. Discussion: The purpose of the repair guide implant is to restore the patient’s good masticatory function. The implant must be steered to a certain degree, following the Spee’s curve or the compensation curve. Based on the above principle, the interactive simulation software can be used to adjust the simulated implant to the true axis of the natural tooth. CONCLUSIONS: The application of interdigital design and analysis technique of mandibular CT image can better observe the structure of bone tissue in surgical field and make scientific planting plan. The safety and accuracy of implant surgery can be ensured by surgical guide.
