目的分析丹东地区2型糖尿病(T2DM)患者骨质疏松症(OP)情况及骨质疏松症相关危险因素。方法测定232例T2DM患者腰椎及股骨颈(Neck)骨密度(BMD),根据1998年WHO制定的OP诊断标准把T2DM患者分为OP组与非OP组,比较两组生化指标、BMD差异,并进行相关性分析。结果 1.T2DM患者中OP组与非OP组之间生化资料比较示OP组低密度脂蛋白(LDL-C)较非OP组明显高,存在显著性差异(P<0.05)。2.OP组与非OP组腰椎(L1-4)BMD比较示两组之间无显著性差异(P>0.05);OP组的总体、男性及女性的平均股骨颈(Neck)BMD明显低于非OP组,两组之间存在显著性差异(P<0.05)。3.T2DM患者中OP组65例,总体发病率28.0%(65/232),其中男性8例,男性发病率12.5%(8/64);女性57例,女性发病率33.9%(57/168),男女OP发病率比较,存在显著性差异(χ2=10.552,P<0.05)。4.Logistic回归分析示:T2DM患者中女性、有微血管并发症(糖尿病肾病或/和糖尿病视网膜病变)、有高血压、血清钙离子低、低体重指数与OP发生相关(P<0.05)。结论 1.丹东地区住院T2DM患者的OP发病率28.0%,明显高于正常人群,且女性发病率高于男性。2.丹东地区T2DM患者中女性、有微血管并发症(糖尿病肾病或/和糖尿病视网膜病变)、有高血压、血清钙离子低、低体重指数与OP发生相关。
Objective To analyze the status of osteoporosis (OP) and related risk factors of osteoporosis in type 2 diabetes mellitus (T2DM) in Dandong. Methods The bone mineral density (BMD) of lumbar spine and neck of 232 patients with T2DM were measured. The T2DM patients were divided into OP group and non-OP group according to the diagnostic criteria set by WHO in 1998. The biochemical indexes and BMD differences were compared between the two groups Conduct correlation analysis. Results 1. The biochemical data of OP group and non-OP group in T2DM patients showed that LDL-C in OP group was significantly higher than that in non-OP group (P <0.05). There was no significant difference between the two groups (P> 0.05). Logistic regression analysis showed that women with T2DM had microvascular complications (diabetic nephropathy and / or diabetic retinopathy), high blood pressure, low serum calcium, and low body mass index associated with OP (P <0.05).
