目的探讨腱鞘巨细胞瘤的超声表现和诊断价值。方法对54例经手术病理确诊的腱鞘巨细胞瘤患者的彩色多普勒超声声像图进行回顾性分析,总结其特点。结果 54例腱鞘巨细胞瘤的患者中,术前超声诊断腱鞘巨细胞瘤45例,45例腱鞘巨细胞瘤的超声表现:边界清晰或欠清晰,呈椭圆形或圆形25例,不规则形20例,不规则形20例均为包绕手指或足趾生长,肿块内部回声均为不均质低回声,后方回声均有不同程度增强。彩色多普勒血流成像,28例肿块内部显示较丰富的血流信号,12例显示点状血流信号,5例肿块内部无明显血流信号。脉冲多普勒可记录到低流速动脉血流频谱,流速3~17cm/s。结论彩色多普勒超声诊断腱鞘巨细胞瘤经济、无创、准确,可作为首选的诊断方法。
Objective To investigate the ultrasound findings and diagnostic value of tendon sheath giant cell tumor. Methods The color Doppler ultrasonography of 54 cases of giant cell tumor of tendon sheath diagnosed by operation and pathology was retrospectively analyzed and its characteristics were summarized. Results Among the 54 patients with giant cell tumor of tendon sheath, 45 cases of giant cell tumor of tendon sheath were diagnosed preoperatively and 45 cases of giant cell tumor of tendon sheath with clear or unclear border, 25 cases were oval or round, irregular 20 cases and irregular shape 20 cases were all around the finger or toe growth, the internal echo were heterogeneous hypoechoic echo were enhanced to varying degrees. Color Doppler flow imaging, 28 cases showed abundant internal blood flow signal, 12 cases showed punctate blood flow signals, 5 cases no significant internal blood flow signal. Pulsed Doppler can record low-flow arterial blood flow spectrum, the flow rate of 3 ~ 17cm / s. Conclusions Color Doppler ultrasound diagnosis of tendon sheath giant cell tumor is economical, noninvasive and accurate, and can be used as the first choice of diagnosis.
