目的分析大理市第一人民医院骨科Ⅰ类切口手术预防使用抗菌药物的情况,为合理使用抗菌药物提供参考。方法从HⅠS系统筛选出2015年9月~2016年2月我院骨科Ⅰ类切口手术病例420份,针对病例是否使用抗菌药物、是否有用药指征、抗菌药物选择、预防用药时机、疗程等进行数据收集,根据《抗菌药物临床应用指导原则》、《中华人民共和国药典临床用药须知》、《国家抗微生物治疗指南》及药品说明书等对其用药合理性进行评价和汇总。结果Ⅰ类切口手术共有420例,使用抗菌药物的例数为89,使用率21.37%。抗菌药物使用合格例数为395,合格率94.05%。在使用的抗菌药物中,一、二代头孢占76.40%。术前0.5~1 h给药的例数为83,占比93.26%。用药疗程<48小时78例,占87.64%。结论我院骨科Ⅰ类切口预防使用抗菌药物基本合理,但不合理用药依然存在,需要进一步规范和持续改进。
Objective To analyze the use of antibacterials in the prevention and treatment of Class Ⅰ incision in the First People’s Hospital of Dali City, and provide a reference for the rational use of antibacterials. Methods A total of 420 cases of type Ⅰ incision in orthopedics from September 2015 to February 2016 in our hospital were screened from the HⅠS system. According to whether antibiotics were used in the cases, whether there were indications for drug use, antibiotic selection, timing of preventive medication, Data collection, evaluation and summary according to “Guiding Principles of Clinical Application of Antibacterial Drugs”, “Pharmacopoeia of the People’s Republic of China Pharmacopoeia”, “National Guideline for anti-microbial treatment” and drug specification. Results There were 420 cases of type Ⅰ incision surgery, the number of cases using antibacterial drugs was 89, the utilization rate was 21.37%. The number of eligible antibacterials used was 395, with a pass rate of 94.05%. In the use of antibacterial drugs, a second-generation cephalosporins accounted for 76.40%. Preoperative 0.5 ~ 1 h administration of the number of cases 83, accounting for 93.26%. Medication course <48 hours 78 cases, accounting for 87.64%. Conclusion It is reasonable and reasonable to prevent the use of antibacterial drugs in Class Ⅰ orthopedic incision in our hospital. However, irrational use of drugs still exists, which requires further standardization and continuous improvement.
