目的调查上海市区中老年骨质疏松症患者的骨折风险。方法对纳入的300例病例采用FRAXTM10年骨折风险评估表,调查患者年龄、性别、身高、体质量、脆性骨折史、父母脆性骨折史、口服激素史、类风湿关节炎史、吸烟史、饮酒史,并测定L1-L4和髋部骨密度;分析不同年龄段主要部位脆性骨折风险、髋部脆性骨折风险以及骨折风险与年龄、体质量指数、骨密度的相关性。结果①300例调查者中男女比例为1∶3.6;女性患者腰椎和髋部骨密度均明显低于男性患者(P<0.05),主要部位脆性骨折风险和髋部脆性骨折风险均明显大于男性患者(P<0.05)。②不同年龄段女性主要部位脆性骨折风险和髋部脆性骨折风险不同,主要部位脆性骨折风险以60~69岁年龄段最高(P<0.05),髋部脆性骨折风险以70~79岁年龄段最高(P<0.05)。③主要部位脆性骨折风险与腰椎及髋部骨密度均呈线性负相关(r=-0.443,r=-0.545),与男性患者体质量指数和年龄呈线性负相关(r=-0.349,r=-0.617)。④髋部脆性骨折风险与腰椎骨密度、髋部骨密度及体质量指数呈线性负相关(r=-0.405,r=0.676,r=-0.19),与女性患者年龄呈线性正相关(r=0.213)。结论上海市区中老年骨质疏松症患者中女性骨折风险较高,且主要部位脆性骨折风险以年龄在60-69岁者最高,髋部骨折风险以70-79岁者最高;骨折风险与骨密度成负相关。
Objective To investigate the fracture risk of middle-aged and elderly patients with osteoporosis in Shanghai. Methods A total of 300 cases were enrolled in the FRAXTM 10-year Cigarette Exposure Bench to investigate age, sex, height, body weight, history of brittle fracture, history of brittle fracture in parents, history of oral hormones, history of rheumatoid arthritis, smoking history, drinking history . The BMD of L1-L4 and hip were measured. The risk of brittle fracture, risk of hip fracture, risk of fracture and age, body mass index and bone mineral density were analyzed in different age groups. Results ① The ratio of male to female in 300 cases was 1: 3.6. The BMD of lumbar spine and hip were significantly lower in female patients than in male patients (P <0.05). The risk of brittle fracture and hip fracture were significantly higher in male patients than in male patients P <0.05). ② The risk of brittle fracture and the risk of hip fracture are different in different age groups. The risk of brittle fracture is highest in the age group of 60-69 (P <0.05), and the risk of hip fracture is the highest in the age group of 70-79 (P <0.05). ③ The risk of brittle fracture in the main part was negatively correlated with the BMD of lumbar spine and hip (r = -0.443, r = -0.545), but negatively correlated with the body mass index and age in male patients (r = -0.349, r = -0.617). ④ The risk of hip fracture was negatively correlated with lumbar BMD, hip BMD and body mass index (r = -0.405, r = 0.676, r = -0.19), and was positively correlated with age of female patients (r = 0.213). Conclusion The incidence of fracture in middle-aged and elderly patients with osteoporosis in Shanghai is high, and the risk of brittle fracture of the main part is the highest in those aged 60-69 years, and the risk of hip fracture is the highest in those aged 70-79 years. The risk of fracture and bone Density is negatively correlated.
