目的探讨前列腺增生症(BPH)伴膀胱出口梗阻(BOO)患者的逼尿肌收缩状况。方法采用连续两次压力-流率法检测71例BPH伴BOO者。将在第1次压力-流率测定中得A-G值,按照Schafer氏BOO分级把病人分成3组:第1组为BOO2级19例;第2组为BOO3-4级24例;第3组为BOO5~6级28例。再将在连续两次压力—流率测定中主要参数按组别进行比较。结果第1组中第2次的膀胱最大容量和残余尿量显著增加,最大尿流率(Qmax)和最大尿流率时的逼尿肌压(PdetQmax)均比第1次显著下降;第2组中第2次的Qmax和PdetQmax比第1次显著下降;第3组中仅第2次的PdetQmax比第1次显著下降。结论本方法能准确反映每个患者逼尿肌的实际状况,为治疗方法的选择提供可靠依据。
Objective To investigate the detrusor contractility in patients with benign prostatic hyperplasia (BPH) accompanied with bladder outlet obstruction (BOO). Methods Totally 71 BPH patients with BOO were tested by two consecutive pressure-flow methods. The A-G values obtained during the first pressure-flow rate determination were divided into three groups according to Schafer’s BOO classification: group 1 was BOO2 grade 19; group 2 was BOO3-4 grade 24; group 3 Group BOO5 ~ 6 28 cases. Then in two consecutive pressure - flow rate determination of the main parameters by group to compare. Results The maximum bladder volume and residual urine volume in the second group were significantly increased in group 1, and the maximum urinary flow rate (Qmax) and detrusor pressure (PdetQmax) at the maximum uroflow rate were significantly lower than those in the first group The second Qmax and PdetQmax in the group were significantly lower than the first and the second PdetQmax in the third group was significantly lower than the first. Conclusion This method can accurately reflect the detrusor in each patient’s actual condition and provide a reliable basis for the selection of treatment methods.
