目的探讨孕11~14w超声筛查诊断胎儿畸形的的临床价值。方法通过对2010年1月至2014年6月来我院正规产检的3990例孕11~14w孕妇,采用GE-E8、GE-730等仪器。经腹部扫查,对11-14w胎儿行胎儿系统超声检查,同时对胎儿各系统畸形进行筛查。结果 (1)正规产检单胎妊娠3990例,产前超声发现异常共71例。(2)11~14w检出异常胎儿46例(64.79%,46/71),其中严重胎儿畸形31例;行染色体相关检查13例,异常9例(3例核型异常,6例NIPT阳性)。(3)中晚孕期检出25例(35.21%,25/71)异常胎儿。结论 11-14w胎儿系统超声筛查诊断有助于胎儿严重畸形的早期发现,做到早期诊断,早期干预;建立早孕期胎儿系统超声检查技术标准,开展规范化培训将对优生优育起到重要意义,但孕1l~14周超声筛查不能替代中晚孕期的后续超声检查。
Objective To investigate the clinical value of screening 11 ~ 14w ultrasound screening fetal malformations. Methods From January 2010 to June 2014 to our hospital regular pregnancy test of 3990 pregnant 11 ~ 14w pregnant women, using GE-E8, GE-730 and other equipment. The abdominal scan, fetal ultrasound system 11-14w fetal line ultrasound, fetal malformations of the system at the same time screening. Results (1) For the 3990 cases of single pregnancy with normal pregnancy test, abnormalities of prenatal ultrasound were found in 71 cases. (2) Forty-six cases (64.79%, 46/71) of abnormal fetuses were detected from 11 to 14w, of which 31 cases were severe fetal malformations. There were 13 cases of chromosome-related abnormalities, 9 cases of abnormalities (3 karyotype abnormalities and 6 cases of NIPT positive) . (3) In the middle and late pregnancy, 25 cases (35.21%, 25/71) of abnormal fetuses were detected. Conclusion Ultrasound screening of fetus during 11-14w is helpful for the early detection of fetal malformations and early diagnosis and early intervention. Establishing the technical standard of ultrasound examination of fetus system in early pregnancy and carrying out standardized training will play an important role in prenatal and postnatal care. However, pregnancy 1l ~ 14 weeks of ultrasound screening can not replace the follow-up ultrasound in the second trimester ultrasound.
