目的：探讨血液透析（HD）加血液灌流（HP）对慢性肾衰患者不同分子量物质的清除及临床症状的改善效果。方法：常规HD中于透析器后串联一只50g装灌流器，观察KT／V和TACUrea及血清尿素氛（BUN）肌酐（Cr）、β2微球蛋白（β2－m）下降率及临床症状。结果：与HD相比，HD＋HP治疗期间患者皮肤瘙痒、头痛等发生率减少、程度减轻，心包积液量减少，KT／V有显著意义的增加，TACure符合充分透析标准,BUNCr和β2-m下降率明显高于HD。结论：HD＋HP对尿毒症中小分子物质清除及治疗中的KT／V和CUrea均较理想，能有效改善慢性肾衰患者临床症状，从而提高其生存质量，该方法操作简便，具较高实用价值。
Objective: To investigate the effect of hemodialysis (HD) plus hemoperfusion (HP) on clearance of different molecular weight substances and clinical symptoms in patients with chronic renal failure. Methods: In routine HD, a 50g cartridge was connected in series with a dialyzer to observe the descending rate and clinical symptoms of creatinine (K), TACUrea and serum creatinine (BUN) and β2 microglobulin (β2-m) in KT / V and TACUrea. Results Compared with HD, the incidence of skin pruritus and headache in HD + HP patients decreased, the degree of pericardial effusion decreased, the pericardial effusion volume decreased, KT / V increased significantly, TACure met the full dialysis standard, BUNCr and β2-m decreased Rate was significantly higher than HD. Conclusion: HD + HP is more ideal for the clearance and treatment of small and medium-sized substances in uremia, which can effectively improve the clinical symptoms and improve the quality of life of patients with chronic renal failure. The method is simple and convenient and has high practical value.
